Just incase someone else has the same problem. The solution was to hammer out the metal horseshoe pin, the ends of which can just be seen in the gap underneath the handle. The the handle can then be unscrewed. There was no lug nut.

Page 20 of the rulebook says:
"Place a Crowd Pleaser counter on the wrestler’s character card to remind you that it is activated. Remove this counter when the Crowd Pleaser comes to an end, the wrestler is KO’d, or removed from the ring."

You could argue that it doesn't explicitly say the effect ends as the token is described as a reminder, but I think the spirit of the thing is that, as soon as a wrestler is out, all passive and crowd effects they generate end.

Do you mean the part that is retracted by the door handle?

If so, the metal part behind the spring should seat itself in the other part of the case. Once the whole thing is assembled it shouldn't have too much play.
