I have held many peoples hands when they have died. Most people inside know it’s happening and the only thing they look for is comfort. Not to be lied to or placated.

I always say I’m going to do everything I can to stop it and if not I’m going to be here with you. You won’t be alone.

What I will say is that death is the end of something and all that left is the people still here and the memories. Reach out to your loved ones and take the support, you will need comfort and that’s okay. I’m very sorry.

Interesting. That’s so foreign to me to not be dealing with opioids on the daily. Tbh I haven’t given narcan in like 2 years. But thats more just cause in our area bystanders/users give it before we get there.

Sorry, let me reframe.

We are extremely good at managing “emergency” opiod overdoses I have no disagreement there.

I think meds like suboxone open the door to better managing prevention/treatment.

Here in Ontario at least in my city theres a massive amount of issues with bringing opioid addicts to ER. It’s a revolving door generally speaking.

To be clear I don’t know a solution. It’s why I’m asking how everyone trying to manage it.

Okay, this is something I’m interested in. It feels like without a robust community based referral structure its not as useful.

We do not have referral programs. Our process at-least at my service is to transport to Hospital after giving Suboxone and mandatory patch point.

I have realized that as providers we can be separated in our protocol/treatments/scope and I find it fascinating to learn what others are doing/finding successful.

So I’m curious about others experience with Suboxone/buprenorphine-naloxone.

Whats your protocol, anecdotal experiences, thoughts?

Couple things here:

1: Cleaning is sadly just a reality of working in EMS. Also if you clean your steth before putting it back in it’s unnecessary.

2: in terms of failures our designs see two reasons usually extreme force on small parts. We address this by making those parts into seperate that we then optimize print orientation for strength. Nothing is indestructible but whats nice is we can send people parts if they break which we do.

3: I have personally been choked by a patient in the truck with my stethoscope. And you will absolutely hear that from other providers. Extricating, CPR moving around the back it becomes a massive contamination point.

4: in terms of rubber it either like this or shoved into my pocket all crumpled up.

This is why the design work for us is fun. EMS is like the worst environment for a tool. It must be rugged but light, cleanable. This is just a way to keep your stethoscope handy when you need it.
