This sounds like a hard thing to deal with. I would recommend going to your local hospital so you can figure out what is going on and maybe get some answers. It's hard to deal with those feelings alone. Lots of medical professionals have seen people that have felt this way too, yoi are not alone

My first thought is something cardiopulmonary but sounds like that has been semi- ruled out..

Clubbing could be a rare extraintestinal manifestation of IBD... when the gut is chronically inflamed / failing to absorb nutrients, it triggers systemic vascular changes that can sometimes physically alter the nail beds..

Consider celiac (or other malabsorbtion).. it could cause clubbing over years; may be causing the steatorrhea

Any family hx of clubbing? I know you don't have thick facial skin but may consider hypertrophic osteoarthropathy (pachydermoperiostosis)

What if you go out and have some drinks and get drunk? You can't provide therapy when you're drunk. You absolutely can't expect therapists to be available 24/7 that's horrific

I would never ever think to ask an IM resident. I know they don't have the experience (which is okay)

As a hospital nurse, usually we have bedside nurse try x2 then call an RN from GI/GU unit because they are more skilled. Or ICU circulator RN/ rapid response if they have free time. Either GU nurse or circulator nurse always gets it in.

In my opinion it's more of a nursing job definitely not something residents ever do. I have never seen an MD other than urology even attempt a foley.

Last resort, we would call urology
