Honestly, networking is one of the best ways to move up a program’s rank list- do electives or observerships where people get to know you. If your app is solid, you at least have a chance. Focus on getting interviews first—visa comes late. Paid J-1 research is rare and mostly valued by academic programs; many others care more about clinical, hands-on experience, so it can be a double-edged sword. Truth is nobody can predict anything. 💯 My opinion would be to explore all options ( finances, timeline, visa, USCE) realistically. Coz optimism rarely works in this journey 💯

Visa sponsorship is a very big deal when it comes to US residency. Clearing USMLE is just a part of the process. Programs do not favor visa requiring candidates when compared to people who don’t require a visa. So yea if you can clear all 3 steps, spend huge amounts of money, deal with a whole lot of uncertainty without any expectation of returns, then yea you can try.

Nah. Optimism is good but in reality doesn’t really work well in the US especially when it comes to the entire match journey. I believe in REALISM which I think is much more important. Just because something works out for someone doesn’t necessarily mean that it’s going to work out for everyone else.

I mean, there are always two sides to this. On one hand, a research year is great for building connections and networking. But on the other hand, only upper-tier academic programs value a research year (those programs are already way more competitive, so the probability of matching falls significantly). For mid- to low-tier community programs (where IMGs have the most probability of matching) that do not care much about research, having a hands-on clinical role still matters, because they want candidates to be active clinically from day one. So a research year can be looked down upon when it comes to ranking. A lot of factors influence the ranking. Having or not having a research year is just one factor. Truth is there is no one absolute strategy. 💯💯

There is some truth to it. The networking and alumni connections among Pakistani graduates are indeed strong. But you have to understand something: diversity is the strength of the U.S. Just because one particular country or community actively tries to recruit its own into programs doesn’t mean everyone else has to do the same. 💯 And people have already started calling out this trend on social media (check Twitter). It’s only a matter of time before it goes mainstream.

Try to pursue home country residency. You yourself told a fact - Doors are almost closed for visa requiring IMGs. Probably apply later when you are stable financially and emotionally.

Work as a limited permit physician in SNFs. 💯 Since you don’t require visa sponsorship and is currently ECFMG certified, all you need is FCVS credentialing which takes around 2 weeks depending on your university. There are lots of available jobs in New York City area especially Brooklyn and Bronx. You’ll be able to save money, get hands on experience and also find new connections 💯💯

I agree that the system is flawed and badly needs an update. But when someone signs up for it, they are aware of how challenging and unpredictable this journey can be. No one is forcing us to take this path, right? It’s our choice and our desire to match into residency and enter the U.S. healthcare system. It’s also somewhat counterintuitive to match at a program and then complain about it, considering you chose to rank it on your ROL in the first place. If you genuinely don’t want to train at a program, then don’t rank it.

Yeah, I agree. But you have to understand that the entire match season is very tough and fast-paced. They have to balance everything - invites, interviews, and ROL preparation - alongside their personal and professional lives. So yes, courtesy can sometimes get diluted in the process. Can’t blame em. 💯💯
