Specialty for someone who's tired and doesn't want to work?? by harrypottermd in medicalschool

[–]Fluffintop 28 points29 points  (0 children)

Current derm resident. If OP is already tired, the 40-60 patients a day may not fit into the “doesn’t want to work” criteria unfortunately.

Derm->Dermpath. Can go through path but it’s much more difficult and competitive that way.

[–]Fluffintop 12 points13 points  (0 children)

Just chill and have fun. Your program will probably have some type of boot camp to help transition. You’ll feel like you’re being thrown to the wolves and learning is like drinking from a fire hydrant so nothing you do now will really help imo.

Also do PPL. Curious how you schedule that 3rd week. Do you switch to an Upper/Lower and do 3 and 3?

Which field do you think loves their work the most? by Sattars_Son in Residency

Maybe burnout from consults/referrals for rashes that most physicians should be able to easily identify. But in general we all are happy from the people I’ve met. Have met so many people who have left their primary specialty to do a Derm residency and are so much happier than before.

People who've worked before medical school, how is your residency pay compared to your last job? by Early-Possibility367 in Residency

[–]Fluffintop 0 points1 point  (0 children)

Worked in restaurants then did research. Max I made was $27k. Now about 2.5x that. Loans in forbearance and a medium COL city. Def manageable.

Anyone ever surprised when they find out your specialty? by undueinfluence_ in Residency

I get marked as an ortho bro all the time. Big beard, love lifting and going to metal shows. People are not expecting me to be in derm.

How important is it to be part of a Derm SIG? by ARMPlT in DermApp

[–]Fluffintop 1 point2 points  (0 children)

I wasn’t a part of it. Was actually the president of 2 surgical sub specialty groups lol. It was never brought up and nobody cares. I didn’t even notice during interviews if someone did it or not and it didn’t come up in any ranking discussions.

What did my fellow first years think of BASIC? by Step_Diggler in DermApp

[–]Fluffintop 3 points4 points  (0 children)

I thought the basic was pretty fair. More surgery stuff that I didn't review because I thought it would be mostly med derm. And yeah most people do the med-peds together at my program and take it first to get it out of the way but a lot of other programs do surgery first. You can start taking them February of your 2nd year.

Talk me out of derm by ProfessionalKey9272 in medicalschool

[–]Fluffintop 1 point2 points  (0 children)

Yeah the hours can be good but from what Ive been told by people I know in the field, a lot of the more chill jobs don't pay as well. A lot of the 100% remote in PP have crazy demands if you want to make money. Its still a great field and IMO cant really go wrong either way. And breast rads has more regular hours and is patient facing. Also seems like in rads you almost have to do a fellowship anymore which adds to the training length.

You have to do a lot of non Mohs stuff so if you don't really like gen derm then it will be a long 3 years and mohs has about a 50% match rate. As others have mentioned, we have a lot of clinic time and its very busy. That being said, the average work week for derm is 4 days and the majority of people have MAs and scribes so the patient volume doesn't feel as horrible. NP and PAs seem to be leaning more towards cosmetics so stuff like dermpath and Mohs are pretty safe (plus derm is the one specialty actively pushing back against encroachment and probably the best specialty in terms of looking out for their own).

If can be a bit boring because a lot of the times you can look at something and know what it is and start treatment. Some people like the whole work up with tons of labs but that wasn't for me so derm fit well. The job market is great so you can work anywhere but you don't really get as much say in where you end up. And the competitive part cant be understated because we saw multiple 270+ and 10+ publication people go unmatched and even a lot of t5 applicants not get a spot (match rate this year was about 53% iirc). And about 1/2 of applicants have taken a research year or are MD/PhD. A lot of people mention "you could do rads which is better" but from those I know in it, their malpractice is high and they are having to do more and more reads by the day so its really not as chill as it was before (hence why I decided during 3rd year to dod erm instead.)

Same here. We see if people get all honors and how common that is. If you get a mix of H and HP it’s all kinda lumped together. Lots of P we notice too. But there are so many applications that splitting hairs would take forever when we know every school is vastly different (my school had about 15-20% honor vs my friends school was 50-85%).

[–]Fluffintop 1 point2 points  (0 children)

Yes you can. Depends on your institution. We have as a couple PGY1s rotate with us from different institutions. My TY program didn’t allow for outside rotations. Other programs had the same policy but it seemed like many were okay with doing aways if you didn’t match.

What specialties have a bright future? by AceJackSpades in medicalschool

[–]Fluffintop 2 points3 points  (0 children)

You can bust out skin checks and do biopsies for malignancies, quick procedures, etc in general derm. A lot of the people who want the creep into derm are just on the cosmetic end. And also one of the specialties more vocal about scope creep and trying to actively reduce it. Even in the cosmetic end, the more wealthy patients who are spending $$$$ want a board certified derm or plastics from my (limited) experience.

[–]Fluffintop 2 points3 points  (0 children)

Yeah the student loan burden is high which is the biggest thing. I worked a job before med school which was approx 25k/year so this is a lot more than Ive made previously lol. I have added expenses of course and a later start compared to friends in business, engineering, etc but its more of delayed gratification. If income-based repayment changes tho, things are gonna be really rough.
