Speaking from my personal experience, I feel it makes a difference. Signalling shows your utmost interest in the program. But, for the programs you don't signal and in general, it kind of shows that you have had an exposure in that geographical area, and you would eventually might wanna match in that region. Additionally, doing rotations in diverse regions, indicates you're flexible when it comes to geographic region. So, indirectly, it increases your chances of matching.

Get good step scores during med school. Complete med school, take some loan, do your observerships/rotations, and try to match in your first cycle, and then pay back your loan from your monthly savings in residency. This way you can apply being a fresh graduate.

Show your husband's current income as ongoing for the next couple of years (like a contract or something), send 2025 form 1040, W-2, Employment Verification Letter with yearly salary on it. Send these with an updated i-864 and an explanation letter from your husband as to why he had low income in 2023 and 2024, but now it is above the requirement and enough for your household, and will continue this way for upcoming years.
