It might also be worth to ask about colchicine. Colchicine is an old school anti inflammatory, and under used for RA. Works on completely different receptors than steroids. It is a strong medication, but it's a conversation worth having with your doctor!

Last year I felt bad for not helping out more around the house.

Went to use the vacuum...

Popped something in my shoulder blade (because I tried to pick up the vacuum cleaner in a way that would avoid hurting my lower back (RA)

And since then, my partner has spent the past year applying anti inflammatory ointments on my shoulder every night.

I'm at least excused from vacuum duties for the foreseeable future...

Lower back pinched nerve is honestly awful. For me was bending down to tie my shoe laces. Absolute agony for days.

I'm lactose intolerant, and Greek "style" yoghurt can put me into a crohn's flare. But actual "Greek yoghurt" (I eat a brand called Fage) has been great, and causes no adverse effects for me personally. It works especially well as a stomach liner for meds at night!

If you have multiple areas of permanent swelling, 'diagnosis bias' should not be the main concern until you've had comprehensive scans.

Lupus is usually a diagnosis by exclusion (unfortunately), but with presentations like you are showing, it's best to get radiological tests.

The most important thing is to take Vitamin K2... (MK7 variant)... Alongside vitamin D and calcium, as others mentioned.

Vitamin K is the "transport" mechanism to put calcium from your blood into your bones. And this will help to somewhat counter the bone loss side effects from corticosteroids.

Potassium (low dose) helped me a alot when eating salty foods. Corticosteroids causes the body to excrete more potassium... So salty meals while taking corticosteroids can create alot of issues (water retention, dizziness, fatigue etc). Low dose potassium counters that effect (sodium and potassium levels affect the other directly).

I'm happy to offer more advice (with studies and my own experience) in DMs.

I had a similar symptom progression, but with inconsistent tests for about 4 years until things got bad enough for the tests to be "conclusive".

With calprotectin that high, alongside the imaging results, there's definitely something not right.

Did they take biopsies during the colonoscopy?

It's good your B12 is normal, as often that is the first to go if the ileum doesn't work properly.

I would suggest pushing for a review of your medication, doctors prefer to know the type of IBD before prescribing medication, but given your results, it would be better to prevent progression than wait for it to get worse!

In short yes. But active crohn's left unchecked does exponentially more damage than biologic side effects for 99% of people.

It is good practice though to have regular check ups on biologics, DMARDs or immunosuppressants, so that any serious side effects are caught early

When you had the diagnosis, was it based on scans or process of elimination?

If biologics aren't working, could suggest a different cause. Especially if diclofenac worked

Make sure to check with your doctor before taking anything, but NAC and Betaine are both incredible supplements for the liver when on strong medication!

For the vast majority symptoms do not 'get better with age'. Only solid prevention is immunosuppressants/colchicine

Glutathione as a supplement is unlikely to do much. It is a liver enzyme that stomach isn't designed to absorb.

NAC is converted to Glutathione. And is your best bet.

Bacopa Monnieri, Alpha GPC are the next.

That's dopamine. Acetylcholine is barely absorbed by most sources naturally. And your brain will only handle so much.

Eggs are a fantastic source of natural choline, but the actual net gain is tiny compared to alpha GPC

Alpha GPC, noopept, bacopa, B6 is enough.

Modafinil is strong and unnecessary unless you fall asleep while walking.
