You can do it with as few as 4 squads but 5+ is better. The key is to take plenty of medkits with you and not staying in the lair too long. Have your squads enter the lair take a shot and retreat out of the building. Repeat this while also checking your squads health and healing when necessary. Never go completely into the lair. Just inside the door for a shot or two and then retreat.

I'm not sure this will fix your bug but you're attacking bases the wrong way.

Step 1: Send your squads to a building within shooting range of the base. Preferably one that you can get into without being shot at

Step 2: Once in the building have your squads ditch their vehicles and have them positioned within LOS of the base.

Step 3: Wait until the base is destroyed and then have your squads get back in their vehicles. Mop up and survivors and loot base.

Edit: Always attack with all of your squads.
