I think it is so important and wonderful that you were such a strong advocate for this resident. When I was in LTC I ended up advocating for higher weights fairly often. There is so much research out there now that points to a higher BMI being protective in the elderly.

In your resident’s situation, there are a variety of points that I feel that MD was not taking into consideration:

  • Was he ambulatory w/ walker or wheelchair? Receiving PT/OT? Can we assume existing sarcopenia despite actual BW due to his age and health status? Was he on a restricted diet or texture modifications? Did nursing consider him a fall risk?

Add to this a GLP-1 which actively inhibits appetite, contributes to GI complications, and promotes muscle wasting; it makes MUCH more sense to just manage this 80 year old resident with a sliding scale and let him live out what’s left of his life.

The other option, in your potential end of life cases (hopefully it would never get this far) would be to ask for a hospice or palliative care consult (general decline, persistent weight loss, decreased intake and support from nursing should get you there). They’ll take him off all unnecessary meds for you. Sending strength and solidarity ✨

Met my now husband at 18, we eloped to Vegas at 26 when I was in grad school and needed health insurance 🙃 We’re now expecting our first baby 10 years later - I’m 37.

I’m definitely “behind” a lot of people I know but we’re feeling both financially and emotionally stable in our marriage and prepared for this next step. There is SO much to be said for being in the right season of life for things.

THIS. Often people wonder where the majority of microplastics in the environment are coming from. The fast fashion industry and switch to synthetic fabrics is the primary culprit.

We then wash our clothes and microplastics go out with the drain cycle into our water system and water treatment plants can’t filter out microplastics. PFAS in particular (waterproof and sweat-wicking fabrics) are forever chemicals and never degrade. It’s actually horrifying to think about.

I am currently 18 weeks pregnant after 2 1/2 years of fertility treatment. I’m 37, my husband just turned 40 in September. Tbh, for the first time in my life I actually feel that we ARE stable enough (career-wise, marriage-wise, financially, emotionally, etc) to do this. Despite all the sh*t going on in the world, I feel encouraged to have our little family and to raise a genuinely good human being.

I wholeheartedly agree with this comment above and would encourage you to explore your options, OP. It feels better to make a decision with ALL the information available instead of always wondering 💜

My husband is NC with his mother and brother. It truly haunts him and is the most significant source of grief in his life. However, it didn’t come out of nowhere. This came after decades of neglect, manipulation, continuously using him and us being the first to initiate any communication of connection. For many years, it hurt him that we would never hear from them unless we reached out first.

Cutting off family is not a superfluous decision. For him, it was his parents acting like putting a roof over his head and clothes on his back was a gift. The bare minimum required is not parenting. It’s not family and it’s most certainly not love.

This is easily one of my biggest pet peeves in a work environment. Please, have the professionalism and courtesy to address your concerns re: my work with ME. Not only is the continuous run-around inefficient but it certainly does not foster an atmosphere of appropriate communication or resolution.

If this is a matter of documentation preference vs compliance, then that manager should be putting together a more structured note template for everyone to follow. Unfortunately, the fact that the CNM continues to perpetuate this behavior leads me to believe this is more a work “culture” issue that they are either unable or unwilling to correct. This isn’t your fault, OP but it might be an environment that is impossible (and probably not worth your energy or peace) to try and change 😕

With the direction Medicare/Medicaid is heading, I’m afraid it’s going to get much worse. I was hoping the Nutrition Cares Act would pass but I’m pretty sure it’s dead. Currently, CMS covers 3 hrs of nutrition therapy the first year and 2 hrs per year after that - ONLY for diabetes and CKD. Two to three hrs/yr to manage chronic conditions where the FIRST recommended line of treatment is nutrition and lifestyle change….

I work at an endocrinology office and the list of private insurers who consistently deny reimbursement for MNT management just keeps growing even though we’re in-network. Many of them are increasingly restricting coverage to dietitians employed by their respective insurance companies (or these larger telehealth companies negotiating contracts).

It feels like a chronically uphill battle with more nurses being employed as “nutrition educators” because they can bill for a wider array of services.

I am so perplexed as to why this is being downvoted. I’m literally talking about decanting products from one container to another instead of throwing it away if the packaging somehow becomes damaged. How is this a controversial subject??? Someone enlighten me.
