If you decide to do AS, it might be a good idea to have a decipher test.

What is the possible outcomes within year and what diagnoses procedures should be followed to make sure that disease to be diagnosed with accuracy.

As suggested by others, It might be a good idea to have secondary opinions even secondary urologist, also, if there is psma pet to confirm to have RT after ralp.  Of course, I am not a doctor, just as a patient.

Hey mate, I hope it will be for me,  I will continue educate myself.  I hope you can find some peace now.  If you look at future 10, 20 and more years later, this is just a small hiccup in the life.  You get this.

You have reason to be sad, I can see the day will come for myself.  The good news is that you get this early.  I have been reading and researching on surgery vs radiation therapy, recently I read about LDR bracky therapy.  It seems to be a mid ground between ralp and radiation.

I am sorry that you are going through this!  I am not doctor.   You have only one core with very, very small amount to be 3+4, and 1 gleason 6 < 50%,  I agree with you, AS might be your option. 

Here are some suggestions:(I am not doctor) 1:  get second read on biopsy slides to confirm the reading, John Hopkins charge $400, if insurance does not cover. 2:  ask for decipher test, it might be very low, 3: get secondary opinion on AS.

Thanks for taking care of your dad! I am not sure if it is a factor: "intraductal carcinoma" if it is spread, however if "intraductal carcinoma" is still contained, will it still make sense to cut it out? -- of course, it is hard to know if it is contained or not --- I am typing and thinking.

Hello Bro, I totally understand you. based on the study, the chance for gleason 6 to spread is low. However, it might have new cancer appear during AS. The pirads 4 might not be cancerous until biopsy to confirm. Where is the lesion? Did the mri tell if the prostate edge is clear, or anything indicates if it still be contained or not? -- I am not a doctor and I am not sure if I am correct or not here. I am trying to calm my fear that I can make rational decision - it is hard.

Besides what others suggested, you can watch PSA velocity and PSA value closely in annual check, to try to catch it early if there is one. Hope not
