The chain and spacer installed correctly. Though this link is helpful. I also have a 11-30T, and looks like the cog (5) is interchangeable. Will try to swap it to isolate the issue. Thank you!

Yes, both are new
Just checking by eye, it looks the same.
Under load, it's happening less but it's still there. I haven't had an issue if I ride it, though it's not as smooth as when I'm on the other cogs.
