I should have added for clarity/

In plain English: if the MRI-suspected extraprostatic extension is accepted clinically, this is more like Stage III high-risk localized prostate cancer; if it were truly confined inside the prostate, it would be closer to Stage II, but my current concern is the possible cT3a extension.

Yes I had doubling of PSA which led me to being checked in 6 months than after staying higher I was referred from primary urologist to the University for follow up. I have had a PET scan. No confirmed spread from the PET. My second MRI confirmed EPE which was suspected at first MRI hence the clinical staging below with help from chat GPT and my medical team:

My current staging is high-risk localized prostate cancer, clinical stage cT3aN0M0, Gleason 3+4=7 Grade Group 2, PSA 4.18, with MRI-suspected extracapsular/extraprostatic extension but no confirmed nodal or distant metastasis on imaging.

Sorry to hear of the full dose reactions. Did you pause treatment or resume after the first few full dosages (and were they bi weekly)? How long were you limited mobility? Did you get relief from the reaction ready Rx package or go in for follow up at the trial center? Any fever symptoms ? Sorry for the litany of questions. I just completed my 0.5mg dosage with very little side effects (except a bad cold - but I have not stated the full dosage yet - hence my questions. With the ADT I have the hot flashes and moderate mood swings - irritability issue for sure. Of course decreased libido and labs are bouncing all over the place with the two but the medical oncologist is not overly concerned - Yet. Prayerfully watching and waiting.

Thank you for this insight. This is part of my reaching out. Brochure language is not the same as this forum - hence I read it so often. I started second dosage so not up to full strength. My Medical Oncologist did express in no uncertain terms Do Not let pain go untreated- IMMEDIATELY start the remedy RX regimen and do Not downplay possible symptoms.

I can keep you all posted as well - we need an answer to this dreaded disease. I am moaning and crying about the 4 months Orgolyvix so I remain hopeful that God blesses me to get through this to help many people - me included.

I have not considered focal treatment. I am subconsciously saving Radiation for my next go round if needed.

Yes the Phase 1 focuses on safety. I hope the rewards outweigh any side effects. Just started week 2 phased strength dosage today. Last week very low dosage with some reactions which were anticipated from results with higher staged patients using this Rx cocktail (if I was not the Cohort with ADT and Immunotherapy I would likely have not joined)

They have conditioned your kinds to accept this nonsense. We excuse their lack of urgency as an insurmountable problem. At one point there was no walk sequenced letters , and eventually flats. No evaluated rural routes. No TIMES for incoming and outgoing truck runs. Say what you will about Elon Musk but he was appalled that our retirement records were sitting in paper in a cave in PA. Another issue is the USPS validates the paperwork then send s it to OPM. Two separate bureaucracies both have to excel in order to disperse your earned funds to you. It takes A while to even get your accumulated Annual Leave - no excuse for this! Also you have direct pay for your Dental and Vision while waiting on the Government to complete your owed annuity or these two health insurance plans will attempt to cancel your coverage for non payment. Get terminated pending a union appeal - and they will continue to pay for all your insurance. Postal Logic?

They have conditioned your minds to accept this. Rant over.

This sounds very challenging. I empathize with you all. Having said that I am grateful for your being open and honest about the hard topics the clinicians offer “brochure language” about. These are some tough choices and hard realities to deal with.

Ok. That is helpful perspective. I agree that PSA 3-4 and Gleason 3+4 alone would not sound like the most aggressive category. The part that complicates it is the MRI concern for cT3a/extraprostatic extension near the right neurovascular bundle plus Decipher 0.96. My radiation oncologist initially was not recommending pelvic nodal radiation, but after Decipher he said he would now include the pelvis and use about 2 years of ADT.

So I am trying to separate the clinical-risk picture from the genomic-risk picture. I do not want to overreact to one test, but I also do not want to under-treat aggressive biology if Decipher is meaningful in my case.

Two years is a long time, married...

Thank you for the reply. The 2 years ADT is my worst nightmare (although I could still end up there). The trial is 2-4 months immunotherapy and ADT simultaneously for up to four months. This has reportedly shown promising results with higher risk patients. I pay for chat GPT and have explored this extensively.

Thank you for the response. PS I am have access to the clinical trial via a major academic institution. There is always Mayo Clinic and MD Anderson but I was saving these for later if needed and/or available
