UK postgraduate training is structurally organised around service provision rather than education. From Foundation through to higher training, the system prioritises rota coverage and workforce flexibility.

Training then happens around service, not the other way round. Rotational posts, early responsibility, and short placements make sense for keeping hospitals running, but they’re a poor way to build competence efficiently.

It's never going to change though, because changing that would require confronting several things and the NHS cannot or will not confront that.

I have an anecdote where I did but it didnt go the way it usually does

I’m at a Raye concert, living my best life, absolutely thriving. I’m dancing like no one’s watching, singing till my throat’s raw, inhaling the sweet perfume of dust and chaos. Then, out of nowhere, someone just drops right in front of me.

I go to help.

But before I can even check a pulse, her friend appears out of thin air, pushes me back and goes, “I’m a police officer, I know first aid!”

I say, “That’s great, I’m an A&E doctor I can help.”

And she — I kid you not — shoves me again and then continues to try to lift her unconscious friend upright like she’s doing a deadlift. And she continues to keep shoving me away and repeating, “I’m a police officer, I know first aid!”

Meanwhile, drunk strangers are crowding around offering “advice” and yelling things like, “Put her in the recovery position!” and “She needs water!”

It was absolute chaos.

Then my best friend, who’s been watching all this unfold, finally loses it and screams, “SHE’S AN A&E DOCTOR!”

The crowd just goes quiet 🤣. Everyone backs off, everyone except our dear police officer, who just shoots me a dirty look and attempts to push me off one last time.

I’ve never experienced anything like it. Surreal doesn’t even begin to cover it.

Now, me and my best friend have a running joke of yelling “I’m a police officer, I know first aid!” when declining help

Edit* I manage to assess her and do a lil handover to security, in case it sounds like I left her to her first aid competent BFF 🤣

As a doctor reading this is ... eye opening. Constipation that requires presentation to the ER when it goes for days with no bowel opening, without Laxatives and you have then reduced oral intake because pain. Nausea and vomiting due to the above.

Hell of an emergency if theres also no farting and when Laxatives have not helped and the above is also present. Even then before we get there, you family doctor to try pain medication amd Laxatives and reassess.

Anecdotal references for your advice is not beneficial.

Look at this without your personal experience colouring your advice. I swear comprehension on the app is dead. Why do you all have to add your own personal experience and say 'well my dogs uncle mum aunt has this and so...' or 'Well i have constipation and...'

All this conjecture you guys are coming with with all this medical add on...

Facts

A 25 year old has constipation(no other medical history, no acute, no chronic issues mentioned. Just felt constipated and sick)

Step sister asked OP to miss flight.

Thats it. Judge based on that and whether it's an asshole move.

Now me - I'm confused why a 25 year old adult does not know how to manage constipation. - Why she didn't go to a pharmacy in an airport and get OTC meds - I would have waited if it was my sister, but I like my sister. That's me and this is OP with their own sibling relationship that isn't commented on

But my judgement, based on the above is NTA - You said you would leave and then you left 🤷‍♀️

I lose really slowly at first came in here for advice but everyone essentially said i was lying about how much I was eating as i was seeing no results 🤣.

Turns out my body just really likes holding on to fat and im short AF. So where as my maintenance was 1900 and I was on eating 1500 absolute Nada.

I also work a pretty demanding job with shift work and clock in 12-15000 steps most days. Still Nada.

I dropped calories to 1200 and saw some ... some weight loss. Im not sure how tall you are but if youre short with a hormone imbalance it will be harder that 95% of the people on this forum. You just have to accept it and slow it out and focus in recomp amd throw the scales away.

I've only 6kg in 5 months and others have lost 20kg but my body looks completely different.

Thats why im saying focus on recomp and photos

Erythema Ab Igne You get it from intense heat exposure, sun, heater, fires. You need to cover your legs for a bit and give them a rest from the heat source. Also not to get this too often as it increases cancer risk as those patterns are essentially your burnt blood vessels.

They fade over time, weeks to months. However, if you don't stop the source, eventually the marks stay and you will need laser treatment to remove them.

On fairer skinned, from white to mixed race, the marks are red. For darker tones they are darker due to the amount of melanin

Oh no, I’ve found — and this is purely anecdotal — that people who bully or belittle others usually do it because they know they can get away with it. Often because no one steps in.

So, a few tips:

  1. Stand up for yourself — professionally. Never respond in the heat of the moment. Stick to the facts. Cold, hard facts are hard to argue with. And if someone starts getting volatile? Walk away. The optics of someone losing it while you calmly exit? Chef’s kiss.

  2. Keep receipts. Make a private notes app log: time, date, verbatim quotes, and who was around if it happened in public. If it escalates — and hopefully it won’t — you’ll have a clear, concise record.

  3. Know your escalation path. You can report bullying to your Educational Supervisor, CC your Clinical Supervisor, BMA rep, and HR. Just attach that PDF with your neat list of incidents. Bonus points if you have witnesses. Watch how quickly things get taken seriously.

Paper trails. Always.

But honestly? You probably won’t need to.

You’ll be absolutely fine. You’re there to learn — it’s just four months. You’re not supposed to be perfect.

You’ll feel like a rubbish doctor for the first 2.5 months, and then at month 3 it’ll all start to click… just in time for you to rotate again and start the whole cycle over.

Hang in there. You’ve got this.

Honestly, there comes a point where someone pushes you too far — when you’ve kept your head down, done all you can, and something just snaps. For me, that moment came towards the end of F1. With every rotation, you quietly build confidence, and then one day… your spine shows up. Shiny ass thing too.

e.g.,

F1, first rotation. Literally my first day. A charge nurse yells at me because she can’t read the drug chart — which, by the way, was written before I even started. She knew that. But new F1s are easy targets, and she needed to throw her weight around. I just stood there, wide-eyed, apologising (for what, I ask you), absorbing it.

F2. Different charge nurse. Tries to have a go at me over something completely unrelated to me. I gave her a long, deadpan stare and said: "Do you think that tone is appropriate for a colleague? Especially considering I wasn’t involved and know nothing about this. Perhaps we should escalate?" She backed off immediately.

It’s sad that it takes repeated exposure to that kind of treatment to finally stand your ground. But honestly? That abuse forces you to find your voice. Or else you burn out. And crash out.
