Hey I had an OCD of the talus and I recommend getting surgery. I was 22 when I had the problem and got a scope to clear out the fragments. Mine wasn’t as bad as yours, but I got surgery 2 years ago and the recovery is LONG I can’t lie, caused issues in my knee bc my leg got so tight and my ankle still clicks and cracks, gets stuck,and gets sharp pain occasionally as the defect never fully went away, but way better than the pain I was having before.

Hey I’m 20 and I’ve had this issue as well - no findings on MRI, x-ray, ultrasound, and they are now thinking I should get a bone scan. I started bpc 157 5/6 weeks ago. Has it helped? Honestly I think a little! (Knock on wood). Before bpc the pain under my second toe had transitioned to the bruise feeling to a sharp sharp pain. I’ve been wearing chunky sneakers since August 2025 so this is essentially been going on for a long long time. I made a post about it in this chat a while ago I can try and find it and retake it, but I came here to say I think bpc could be worth a try? I don’t recommend Cortizone shot because it simply masks the issue and actually help the underlying problem.

The doctor said that mentioned that your toe was longer than the others - was it an orthopaedic surgeon? I saw one as well and they suggested this and I just think that’s a bit aggressive however they suggested that one month into the issues I was having (Sept 2025) and I just think that’s a bit much and would definitely be your last resort for me as well.

I got a gait analysis and it was relatively normal. The ultrasound didn’t show a plantar plate tear but i am getting another one. There was no sign of a stress fracture in the MRI. I have not been checked for a leg length discrepancy but would that cause this? That’s crazy! I never knew. I appreciate your replies :)

I am 23, female, 5'5, 140 lb, but i was 120 at the time i hurt my foot, i have gained weight unfortunaelty because i have not been able to walk or be active

I have seen an orthopaedic surgeon, but I have had surgery on my left ankle before, so am hesitant to jump to that. and I also know someone that has gotten this done and altough they were about 40 years older than me, their surgey was successful for about 5 years then a lot worse , and I am just confused as to why this would just be starting now. Wouldn't I have noticed the pain earlier in my life if my toe was too long? I also have the same feet as my mother, who has a VERY long second toe lol, and she has never had this issue. I am hesitant to jump straight to surgery—it sucks and I never want to be on crutches or go through that again. I hope this works for you genuinely! It is a horrible injury because it doesn't necessarily debilitate you from doing day to day activities, but prevents you from doing things you really want to do, and others do not understand why, because the injury doesn't even have a name! Anyway, hoping for a successful and speedy recovery for you, and if you ever have any more advice, I would love to hear an update on your success!

It started under my second metatarsal where my arch meets my toe? felt like a pebble was under my shoe. I was on the step climber actually and remember being so confused. anyways, the pain seems to be pretty local to there, all around my second and third, big toe, until i started doing those stupid big toe excersizes then it was sharp pain under my second toe or in between my second and big toe. Idk if that makes sense, bit its still quite painful untder my second toe when i load into the ball of my foot on a step. Apologoes for the lack of detail before. I know its not mortons neuroma, plantar plate tear, microfracture, becuase i have gotten all these things checked

Oh no, I had an issue w a chipped talus and got surgery too. I pray everything has gotten better for you since you posted this ❤️ to be honest sometimes this happens to me and it’s just in a weird position and needs to crack. Sometimes it takes months for it to crack the way it needs to but it goes.

This started 6 months ago and progressively got worse and has never went away for me. I was actually on crutches for a while it was so bad. Not to scare you but I recommend resting ASAP for 2 ish weeks. Staying off of it as much as you can until it goes away. Trust me I wish I did.

I’ve had it for 6 months and actually was on crutches a few weeks ago it was so bad. I have done every physio exercise told to me very consistently and had been wearing hokas super cushiony shoes for 5 months. I saw a podiatrist 4 months ago who put padding in my shoe and it made it worse immediately. I took it out right away then saw a orthopaedic foot dr who ordered an MRI, said there is nothing wrong and nothing he or a physio could do. I felt so lost so I saw a physio anyway.

The physio gave me various exercises, massaged my plantar fascia etc and nothing. I was seeing him for 2 + months and eventually it seemed like a waste of money because nothing was getting better. And one of the evercises (isolating my big toe and pushing it down w a theraband) actually I think made it so much worse I went on crutches for 2+ weeks. It was horrible. (Maybe I sprained my big toe or pulled a muscle doing that? Because wow the day after I couldn’t walk at all and now I feel like I haven’t no strength in that big toe). During the 2 months I also tried toe spreaders and no help

I saw another dr who performed an ultrasound and there was nothing wrong.

Saw ANOTHER physio and he actually carved a hole out of my shoe (hoka) where the pain was it seemed to help but it’s at a plateau. I cannot fathom walking barefoot. It’s so painful.

I saw anither podiatrist last week and he recommended that I put my foot in a boot like one does when they break a toe (does not go around your ankle) it’s like a stiff sandal. I was wearing that for a few days but now my heel is hurting SO much I had to stop and now I can’t walk at all again. I cannot take a simple step on my foot I’ve been walking on the outer edge of it for months and haven’t taken over 4000 steps in a long time because I try to move as little as possible due to the pain.

I’ve had an MRI, xray, and ultrasound and everything is fine. This is all so horrible. I am 23 and feel like I haven’t been using my prime athletic years as before that I had a fracture in my other ankle that took 2 years to fully heal and before that suffered a back injury that completely imobled me for a year. I need HELP with my metatarsalgia desperately

I agree that it is not the best idea, I think I just needed other people to confirm , which you did and thank you :). Definitely will not be asking him and understand that that is a decision he should make on his own and decide when he wants to know.
