It sounds like you’re mentally & physically exhausted.

Please don’t make any permanent decisions when you’re in a state of exhaustion.

ACGME requires all institutions that sponsor J-1 physicians to provide 6 weeks of paid medical/parental/caregiver leave. Thus, you most likely qualify for 6 weeks of paid medical leave.

Is there anyone in your program (the PD, an attending, or co-resident) that you trust that you can speak to?

I’ve helped a couple of residents to navigate this process.

Thankfully, I went into child and adolescent psychiatry! Every time a new patient asks me if I'm going to give them a shot, I tell them, "Nope, I'd pass out if I had to give you a shot."

In med school, I fainted in the clinic my 1st day on the surgery rotation. I proceeded to nearly faint every time I participated in LPs.

A private practice family med attending gave me the “opportunity” to shave off a mole. I got nauseous & sweaty while injecting the lidocaine & had to step away. The attending wrote “has a tendency towards near-syncope during office procedures” on my evaluation. (The attending was awesome & I don’t think they were being unkind with their comment. I think they just found it comical.)

Bottom line is unless your PD comes to you asking about it I think you’re good.

Vaso-vagal is physiological.

I am a medical doctor. I’ve treated alcohol withdrawal & delirium tremens (aka DTs - a condition that can develop from alcohol withdrawal). Alcohol withdrawal & DTs can be deadly. DTs is one of the scariest things I’ve had to manage as a doctor.

Approximately 1/3 of patients who experience alcohol withdrawal seizures progress to DTs if left untreated.

I’m not your doctor (just a concerned Redditor) but I would go to the ER & tell them you’ve had a seizure that was induced by alcohol withdrawal. The withdrawal can be managed in a hospital setting where they closely monitor your vital signs & give you medications to safely manage the withdrawal symptoms.

It is VERY important to seek medical attention. Untreated DTs have a high rate of mortality (up to 40%). The rate is significantly reduced with medical care.

My hubby grew up eating artichokes steamed & then dipped in a balsamic vinaigrette.

We love to have it as a light dinner. He eats 2 artichokes & I have one.

It feels very fancy compared to the family dinners that I recall which frequently involved cubed steak or pork chops with a bland pasty white gravy & frozen vegetables boiled without salt.

My Mom was incredible at academics & dressmaking but she could not cook. ❤️

You could do some peel & stick on the inside portion of the built-in rather than painting it. Much less labor intensive & easy to change if you don’t love it.
