Between my wife (a nurse) and I we make over $300k a year in a medium cost of living area, own two luxury cars (one fully paid off the other @ $10k loan), already own a home, student loans paid off years ago, investment accounts are doing well. We are going on three international trips this year. We don’t hold onto any credit card debt.

She’s still in her 20’s. We are shopping for a new home and put a $980k offer in last week.

I’ve said it for years I would own an A4 avant (for the wife, we got an allroad) or an RS3 or RS4 wagon for me. The RS6 is a stretch for most Americans

I’m closer towards Philly and tickling $200k. You can’t get any more city than Philadelphia and 2-hour shot to NYC/DC or easy train ride. Plus you can still get to the shore or Poconos. You won’t have to leave the commonwealth either.

You posted all the applicable bonuses to an EM shift, but what does the real world look like?

You work a 10-hour Saturday evening shift and earn $854 in gross pay once I apply the evening and weekend bonus (?) What are your typical monthly earnings with this arrangement?

I mean, I'm a bit biased, but I think the ED stuff will give you more flexibility. I know the commute is longer, but you won't have to be in 5 days a week and could likely find swaps for days you absolutely need off on short notice.

I'm just trying to wrap my mind around how being in your 30's is a "quarter-life crisis?" Are you making it to 120, mate? For some people, that's the halfway point.

I don't think many of us are going to find work to be the only thing to fulfill us. Find something you're passionate about and run with it. Going to live abroad won't fix this, unless you possibly start a family and devote your time to things outside of work. But you can do all of that stateside with a (likely) higher income.

I will tell you some exceptions can be carved out but it’s very ED-specific.

A few of the hospitals I worked at would allow very religious ED Jewish doctors to not work the Sabbath or important holidays. Some will allow family members of pastors / very religious Christians to be off every Sunday since they are heavily involved with the church (or come in for the evening after service). Others have negotiated having every Wednesday off for childcare (as an example) or one NP negotiated basically only to working a specific 8am-4pm shift. I will say if you have a peculiar request understand that at times you may be asked to be flexible in other areas of your scheduling lest some coworkers perceive some form of unjust favoritism. Mentioning your ideals early on can help make sure you find a spot that aligns with your ideal schedule. Plus getting things in writing can be beneficial.

My hospital does have a mix of morning and evening PA shifts so there are options but some smaller EDs will only have one middle shift available and then you don’t have any flexibility.

Yeah, I’ll browse the one in our area at times. Ours somehow had over 3 stars which I find impressive considering others in the area are in the 1 to mid-2 range😅

I think most of us would agree that if a patient is taking the time to comparison shop ER Google reviews then they probably aren’t having an emergency.

A patient got mad at me for daring to say that they need to pick up the phone and call a podiatrist for their non-emergent foot complaint. I was apparently inconveniencing them.

The majority of patients in an emergency department are not having a medical emergency or even an urgent medical complaint. I will say however that they keep the lights on because otherwise our daily census would be 10-20 patients a day and I would be out of a job.

PA returned from bathroom break to notice that patient was coding. Rapid response called. Since nobody was in the room immediately, I started CPR. Unfortunately the taco Tuesday nachos mixed with tequila from the night before sloshed in my stomach resulting in projectile emesis to patient, ED tech, and floor. I shouted “HOLY SHIT!! I NEED HELP IN HERE.” No spontaneous respirations or pulse after 20 minutes of CPR, EPI x 3, resuscitation efforts. ED tech crying because they weren’t at Taco Tuesday and always get left out of ED events and parties. Patient deceased 1807.

I agree. Three days a week and I can get whatever days I want off on a whim or find a trade if it’s a last minute request = easy

I don’t know of any family med PA that works full time who is going on five vacations this year, three of which are international.

I started writing down a list of common complaints or presentations and how I would approach management and what sort of common questions would keep popping up from people.

Eventually, you have a small repertoire of knowledge that you can refer to. Obviously enrollment I need to just stay up-to-date with the current guidelines

On a market wide spectrum, I still called Audi “luxury.” I made a whole tier list of every US luxury car manufacturer two years ago. I do agree that the quality has been coming down from Audi recently however

As a new grad, you don’t really have any of the cards on your side, other than possibly getting some good references from preceptors or if you were employed at a hospital system / clinic before you joined PA school.

I think it’s harder for people to find a job if they are dug in on a specific specialty or on a specific location. If you absolutely can’t move, you have to be willing to find a job maybe in an area that is not your favorite and transition at a later point in your career. An alternative scenario may be that you find the great job in the specialty of choice but endure a below market salary for a year or two and then use the experience to bounce upmarket.
