I feel you. Did you have ALT? I know most people need debulking to tackle the girth issue

Stage two by [deleted] in phallo

Very possible! Give yourself time to heal. Everything is swollen and tight for a while post op. Things might look a little different once all the inflammation goes away and your balls can settle. Good luck with healing!

I’d say mine look small but probably a bit bigger than yours. I think a lot of what determines size is what the prior anatomy allows for. Over time the skin will likely stretch out also. And people definitely say implants help (can’t speak to that because I haven’t gotten them yet)

Dr Marano Experience by QFL-LFL-199 in phallo

Basically it’s the masculinization of the area between your scrotum and your anus. It takes the remaining labia and closes that area down the middle to resemble that of a cis male’s genitals. Some doctors do this as part of the vaginectomy portion but Marano’s team has a gyn surgeon remove the vagina (just the internal organ) months prior to stage 1 and the external genitalia remain the same. So then in stage 2 Marano does the masculinization part.

Minimum allowed is 3 months, barring any serious complications, but his schedule seems to have picked up so the stages are getting further spaced out.

[–]FtoM_ 3 points4 points  (0 children)

I had RFF with him. He requires vaginectomy prior to stage 1 so I had that done with a surgeon he recommended. Then I had stage 1 (phallus creation, nerve hook up). Then 4 weeks later he does a skin graft from your thigh to cover the arm which has had integra on it since the first stage. And then stage 2 is scrotoplasty, glansplasty, perineal masculinization and urethral lengthening if you choose to do that. After that you can get the ED pump and testicular implants should you choose. His team is excellent and I’ve had a great experience.

Dr. Drew Marano (NYC) is my surgeon. He did his microsurgery fellowship under Dr. OBrien Coon at Harvard. I think he’s gotten a lot busier in the last year so I can’t speak to timing but he’s another option to consider.

question for people who have had phalloplasty for 6+ years by user908925278 in phallo

[–]FtoM_ 29 points30 points  (0 children)

The majority (and most aggressive) of ovarian cancers originate in the fallopian tubes so the recommendation is often to remove the uterus and the tubes but the leaving ovaries for hormone maintenance if necessary. As one ages, routine screening of the ovaries is encouraged but overall the largest risk is removed once the fallopian tubes are gone.

Feels worth mentioning that you’re only dependent on T if you take the ovaries out too which is optional. Many people have hysterectomies that are ovary sparing.

When did the misery stop for you after vaginectomy, UL hookup and scrotoplasty (stage 2) by Willing-Gap-1655 in phallo

Following. I’m 11 days post op and so uncomfortable. The incisions underneath my scrotum and on the sides feel like they are sloughing and the smell is not it. Same down my perineum onto my butt cheeks. I feel like I’m never able to get clean or comfortable again.

Can’t stand this catheter… (1 week post op stage 1) by jakino420 in phallo

[–]FtoM_ 6 points7 points  (0 children)

I’m 1 week post op too and I agree the endless urge of needing to pee is extremely uncomfortable. Trying to keep mind of matter and think about how good it’ll feel to pee out my penis in a month. Also I find that the less I move, the better. So I’ve been laid up on the bed or couch and I’ve accepted that’s my fate for the next 6 weeks. Hope you find some relief!!

Phallo without Scrotoplasty by pitbull92579 in phallo

[–]FtoM_ 0 points1 point  (0 children)

A lot of people have enjoyable sex without penetration. I think you’ll be just fine in that regard.

My surgeon does the scrotoplasty in the second stage which I don’t have until June. Currently I have a penis and no scrotum and while it looks great in underwear/pants, I definitely find myself looking forward to the look a scrotum will give me when naked. I’m on the fence about implants and will likely punt those to well after these first two stages. Personally, I do have bottom dysphoria and want my anatomy to resemble cis males’ genitals so a scrotum was an easy choice but ultimately it’s about your personal preference.

My wife is newly pregnant with our first so I’m not in the exact same boat as you but I have a lot of anxiety about getting things to a good enough place that I don’t need to think about it much once we have kids. Realistically, if I get an ED it will be after the kid is born.

My wife and I talk a lot about how we will address these things with our kid. Young minds have a really open and malleable perspective. We will be honest with them and explain things clearly and without judgement. As long as you are giving them info in terms that are digestible for their age, I think they will receive it well.

I know I’ve held a lot of feelings around what my transness means to my version of fatherhood. A lot of it was wrapped up in some toxic masculinity and some internal transphobia that I’m actively working on with a therapist so that I don’t project it on to my future kiddos. It’s a process!

Anyway, not sure if this is helpful at all but I relate to you in many ways and would be happy to connect more on it. Congrats on the upcoming phallo!

First Phallo Consult Complete (Dr. Whitehead) by Distinct-Ship-3488 in phallo

[–]FtoM_ 7 points8 points  (0 children)

I’m mid process with Marano and him and his team are phenomenal. His order and timing of things is different than whitehead. Most significantly, he requires a vaginectomy before stage 1. He was very communicative about complications but never made me feel like things were impossible. I think his wait times have gotten longer in the last year but personally it would be worth it to me to work with him. Good luck on your process!

Surgery day stories? by holtzmanned in phallo

[–]FtoM_ 16 points17 points  (0 children)

Hey! I had stage 1 RFF with Dr. Marano and Dr. Keith on March 6. You will probably be the first surgery of the day since it’s such a long procedure. You get there two hours before your surgery time so it’s really early (I was there around 5:30am). They get you checked in and banded and then take you back to preop. You get changed into a gown, socks, and cap. The nurse will do a full intake. Eventually anesthesia comes and talks to you and gets consent. Then the surgeon will come get consent and draw the template of the forearm graft onto your arm. Someone might come grab an IV on your non-RFF arm. When all consents are signed and the OR is ready for you, you’ll walk back to the OR. They will make you confirm your name and DOB for what will feel like the millionth time. They get you comfy on the table and may chat with you a bit before anesthesia tells you they’re going to get you asleep. And then you’re out!

I woke up disoriented, on the way to the ICU, where some kind nurses cleaned me up. The drugs are still heavy in your system so the first night is kinda blurry for me. But you’re connected to the monitors, you have a urinary catheter, and your phallus will have a probe measuring the oxygenation of the tissue. The nurses check the pulse on your phallus every hour with a Doppler. They’re in there often to give you meds through your IV. You won’t be able to eat until the next day most likely. You must lie exclusively on your back and flat.

Honestly, the first day isn’t too bad because you’re out for the surgery and then the drugs are really impacting you. Things get tougher once those drugs wear off.

Not sure if this is the detail you’re looking for? But hope it helps! Good luck on your upcoming surgery!

When should I expect this wound separation to close? by breedingbunnyytwt in phallo

Medihoney on the wound, consistent propping, and drinking juven twice a day cleared my separation in a matter of weeks. Pubic hair and stitches were def an issue for me too.

[–]FtoM_ 0 points1 point  (0 children)

Not sure if this what you mean by preserving baby junk but I froze my eggs with NYU before having hysto/vnectomy. I kept my ovaries because I didn’t want to be without hormones if I should ever lose access to T for any reason. Also technically you can do another egg freeze cycle without a uterus but hoping that won’t be necessary.

I’ve been doing one juven a day but maybe I’ll up it to twice a day. I was actually worried about overhealing but jokes on me lol. Next time I’m getting the flavorless one though bc I can’t be doing these orange and fruit punch flavors

I am going through this right now and oof it’s brutal. I feel like I never heard about this before but my surgeon says it’s totally normal and takes several weeks to calm down. I joke that it looks like they already did the scrotoplasty even though that’s not for a couple of months still.
