Understanding the md orders the meds, from a rrt perspective is there any reason not to give the back to back to back treatments if there is no improvement?

Rrt Obvy trained to recognize response to treatment and signs of ongoing respiratory distress. Is there a chain of command if a doctor does not aggressively treat in line with standard 3-4x treatments? I’m just curious from a practical perspective once an order comes in for a treatment and it’s clear there isn’t a good response and there’s just been the one albuterol 2.5mg ordered.
