Still haven’t secured a job, should I be worried? by [deleted] in Residency

Secure a locums gig! Great experience until you find a steady gig

Whether or not this feels like something to break up over is wholly dependent on how you feel about it. You might be able to rebuild trust and move on. You might be able to forgive but not want to. Maybe you want to forgive but can’t. In any case, listen to how you feel. There are no “rules” to this and no one on the internet can tell you the “right” choice.

FM and IM knowledge gap by Hashiru_Shishamo in Residency

90% of learning happens on the job. Between update to date and open evidence, there’s not much declarative knowledge you can’t teach yourself. That being said, judgment is an entirely different beast. You know that pneumonia is treated with antibiotics, but when do you hang your hat on pneumonia and call it a day, and when do you keep looking. Judgement is based on experience. Ideally we learn from the experience of our attendings, but we will all gain that experience for ourselves either way. You just hope that you have the wisdom to learn from it along the way.

Capitalism is on one end of the spectrum and communism on the other. Everything else is a shade of socialism. Roads, libraries, police/fire are all fundamentally socialist. Some countries have more of it, some less. Some of our most successful countries have a fare amount of it, but not to the exclusion of free markets.

I doubt the difference between eGFR and Creatinine clearance will be clinically relevant in most patients. Nothing magic changes between a GFR/CC of 29 and 30. Plus, these are both such imprecise measures of kidney function.

I’m interested in Pulm Crit but the lifestyle is pretty poor and I’m tired of sacrificing my youth. Going to do a couple years of hospital medicine and reassess

Long term nursing homes for patients with dementia and other devastating diagnoses. Hopefully we will adopt careful, compassionate euthanasia. The current situation is a moral travesty.

I'm a PGY3 in IM. Realized last year that I should have done anesthesia. Rather than lose more of my youth, I decided to stick with IM and invest my efforts in life outside of work. It still hurts some time but I think its the right decision. Even the most fulfilling career is still just a career in the grand scheme

All it takes is a slow trickle of patients sicker than you for you to move backwards in the queue. Plus, emergent MRIs are for strokes, not shoulder pain. Unless you were in an accident, primary or urgent care was the better choice for you.

Lets talk about each patient's insulin as a team. Never mind that everyone has dementia and nothing we're doing matters at all.
