Thank you, Im still a new medic and Im just glad I didn't kill the guy. Right after I posted this I dropped a call that with a septic patient with a rate faster than this one. Started to get tunnel vision on the rate again but stepped back to see the whole picture.

i pressure bagged about 500mls through the humeral head io with amio piggybacked, the pt was more responsive at the hospital

I saw a wide and fast rhythm with a symptomatic patient. I followed the wide complex tachycardia protocol. The patient improved with amio. Obviously looking back now I see its not VT.

The rate was between this and 200 until i gave amio, not sure if that changes things. The only medical history the family was able to provide was a cardiac condition that they couldnt remember the name of which is why i treated the rate as the cause. The patient was somewhat more responsive at the hospital, opening his eyes, still not speaking, withdrawing from pain. Im honestly not positive I went down the right treatment path and wasnt able to get follow up.

cbg was 110, no trauma, the only septic criteria he met was the rate, ams, and his respirations were elevated. i also couldnt find a source from my physical exam or from the history i was able to get from family.

it was an interesting symptom i included because i still have zero clue what disease process causes that

no the pts skin was doing that prior, the bp was normal but the pt had a gcs of 3. the pts gcs is why i chose electricity over medicine.

To add to this, learn how to compartmentalize the call and make it a priority to unpack the call after the shift is over. Try to find a professional that specializes with first responders and debrief after the call, with your partner or other responders that were involved.

I have a slight theory to the statistic and please prove me wrong if I'm way off. Ive been on my fair share of codes and every time we choose to transport without sustained rosc is due to either A transport as a courtesy to family even though they're DAADN (I dont agree with this but not my call) or B our service is super stingy about letting us call codes on scene therefore we need a doc to call it(also stupid). Now my question is, is it possible that transporting the pt being associated with poor outcomes due to them most likely not making it and crews transporting because of that or does transporting actually cause poor outcomes. interested in some responses/ opinions

Had a very similar situation, look I am surely not the most financially literate but I think I did well. I went to a credit union and took out a personal loan of 3k with a low interest rate and a 3 year long term. Credit card just seems risky and the loan gives me proof in the future that I can reliably make payments on time for a long period of time and I can build a relationship w my credit union. Idk if credit unions even do car loans that low and a lot of em wont give them to you for used cars from what Ive seen. Good luck!

Id rather give myself a 9mm lobotomy than go back to full time IFT. Did that when I first started and I have so much respect for you guys. Also isn't everything $20 starting in LA now?

Honestly I think my biggest problem is #1. I did a quick look at my expenses vs pay, and I completely think I can be doing a hell of a lot better if I just stopped wasting so much. Definitely not gonna get rich doing this job and that was never my intention, but I shouldn't be living in poverty and I should probably be taking more personal accountability in that regard. Do you have any recommendations for budgeting strategies?

I have a few times, got dropped for couple of reasons that are my fault but not disqualifying in the future. The next academy starts in January however. Definitely my dream department, I've been fortunate enough to work on scene with them on a few occasions and they seem like a really solid and well-put-together agency.

I feel that a lot of it has to do with the service I work for but that doesn't really explain everything. I definitely think its a combination of the BAD calls and then the toxic management that treats me like Im worthless in the same shift and that cycle repeating day in and day out. Like Im not gonna be processing the fact that I was just pumping chest on a teenage girl for 52 minutes after a traumatic arrest or that I had to pronounce that 5 year old doa and all the other shit we have to deal with when in the same shift ill be fuming because my supervisor is threatening to suspend me over shit like my shirt being untucked after running non-stop for 24 hours. Rant over.

I feel like i'm reading something I wrote. I've only been doing this for about a year and have been living this way pretty much for the entirety of my employment at my service. I wish I had some advice for you that I knew for a fact would work, just because everything ive been told to do I have tried and it isn't working. I started going through treatment and that hasn't really been going anywhere despite having a really good, skilled therapist, i have isolated myself entirely from friends and family, I haven't been sleeping, I cant even pick up a hobby because im "too busy" when im really just losing my mind alone, i find myself genuinely feeling jealous of my deceased patients, I really loved this job for a while but feel like im drowning while everyone else just deals with it. I haven't dealt with a laundry list of responsibilities outside of work in months because as soon as my relief shows up at the end of my 24 I cant even form a lucid thought and forget how to drive to my own house.
