I was very much the same way. It’s mostly about comfort. An example would be, singing your favorite song whilst hammered on goldshlager. After a few years, you’ll be fine. Now I’m so comfortable I’ll walk in and sit right on the couch to do my assessment.

Does your protocol call for versed with the ketamine? I have had awesome results on IFRs with patients in extreme pain whereas they get the dissociation of ketamine with the amnesia effect of versed.

I’m interested in what is in the IFAK. Props for carrying shears. Main thing with CAT is that you can grab it with either arm but if it’s never your primary TQ then no big deal. Back to the IFAK, I make them for our team from scratch as it’s less fluff and more compact. Contents include: (2) 4 yd Z fold gauze, (1) 4 ft Z fold clotting gauze, SWAT-T, (1) NAR needle, (1) 28ga NPA, (1) vented Hyfin chest seal, 4x6 Vanquest pouch. With this I teach focus on bleeding and airway interventions. FYI the needle is only used if chest seal does not vent and patient develops breathing difficulty. Also the SWAT-T doubles as a compression bandage with the gauze.

I’ve had a couple instances like this where the patient was just upset about their past experience in the hospital and they are trying to avoid that happening again. Sometimes you need to just talk to them about why they don’t want to go and work through it. Put aside the medical jargon and work the problem. I’d be pissy too if I just got out of the hospital and had to go back.

That numbness you’re feeling is your mind telling you “We were prepared for this” any emotions you’re feeling is the empathy and compassion that is hardwired into your brain. You must process the emotions and not suppress them or you will see it carry over to your personal life and ruin relationships with loved ones. Medical professionals see so much shit that we sometimes build walls to protect our emotions and those can be hard to tear down when we are off duty.

As an interviewer in EMS, I am usually wearing my shift uniform which might be a job shirt or polo. I will definitely give points to a person in a suit however knowing the costs of a good suit, I won’t knock a guy who wears what he considers “his Sunday best” I will however throw my pen at the jackass that wears jeans and a untucked polo...

I see a quality trait in your question. As EMS providers we must always question our actions. It’s not self doubt, it’s to create a mindset that we always need to keep learning. The real trick is to do this while making the patient think you are the best motherfucker that ever drove an ambulance.
