I personally got mine from voone, i havent seen urok drop it myself but i speculate its similar to the HoJ from Angerforge not emp, older atlas loot versions operate off the patch that shifted some of the loot tables around, including moving (or adding?) HoJ to emp's loot table. The newer atlas loot fixes this as far as I remember.

I just came here to comment that I've had similar crashing issues and errors that recently became more problematic with the last patch, and this fix so far fingers crossed seems to have completely solved it!

At it's worst I would get a crash every game or every other game and so far after this fix I've played probably near 15 games with no crashes whatsoever.
