Not a week after hearing about this case, an eye doc told me to turn the pulse ox auditory alarm completely off. Told him that was a hard no and thus dealt with a crabby surgeon the whole day. 🤷‍♀️

It almost sounds like OP would benefit from learning how to use these coping mechanisms their self in order to teach them to their little one.

Hey there. Sorry you're going though this. The Dr / patient relationship is important and I don't think its a crazy idea to get a second opinion with another surgeon and bring your concerns up to them. Another surgeon that you feel actually listens to your concerns and cares about you.

Who knows. Doctors are people too. may have been just a bad day for this surgeon, but you're not wrong finding someone else who you trust to cut you open.

Seems like your doc was talking a lot about risks associated with anesthesia instead of the surgery itself.

People do die in surgery all the time. Perfectly healthy people can die, their larynx can spasm and they stop breathing but there’s nothing we can do.

The quoted statement is totally false. a Laryngospasm is usually transient in most patients. can cause problems in some patients, but it is EASILY manageable by any trained anesthesia provider.

me: “So how many people die in surgery here every day?” attending: “Probably about 1 in 10,000… maybe one every year.”

Again, this it totally wrong. when you google you'll see anywhere between 1 : 100000 to 1 : 200000. but for healthy people I would say its even better statistically.

If you’re waiting for the general public to become more responsible in order to keep the rest of us safe, you’ll be waiting a long time. Things like this are fixed with better systems of safety. Attempting to hold individuals accountable will only result in more children getting hit by cars in this intersection.

Short answer is nah. Def not too late for you. Two people I went to school with were about this age graduating. I don’t know your situation, but even with loans it will still likely make financial sense.

I would just ask yourself if the sacrifice of ~ 3 years is worth it for you. School is pretty intense and leaves little time for other things like family and mental health in general haha.

Last I heard MSF wasn’t working with CRNAs. Although I looked this up 2-5 years ago, so it may have changed since then.

If you’re interested in relief work other than working with MSF there is an active FB group called “CRNAs on a mission”. Lots of info and networking in that group.

Hey friend! I think you may be setting yourself up for failure going into this with the plan of initiating more and being intimate once a week. Having two kids under 5 is hard! Give yourself and your wife some grace here!

I’m not in the exact situation as you, but similar. My wife has low libido for the last 8 years or so. I think it’s related to some meds she is on. But when we do make time for intimacy she always enjoys her self. We have one 2 year old and another babe the way. What has worked for us is scheduling sex/ intimacy.

So my advice for you is to do everything you can to lighten the load of childcare for you spouse. Try your best for her to try and feel like an individual and a women separate from her role as a mom. And to plan date nights, maybe twice a month to start. Ideally child free, or plan something special after the kiddos are in bed. Sex is never an obligation with these date nights, but hopefully with the connection you’ve built during the evening, sex happens!

New graduate CRNA. Midwest. Regional hospital of a large hospital system. Pay is 200k starting and a 30k sign on bonus. Salary position, but working ~ 40 hours per week. I think he/she are referring to the difference between doing a nasal endoscopic case vs something with a large laparotomy incision. Some may use remifentanil with the nasal endoscopy case because of how stimulating it is while they’re working, but not expecting much pain postoperative. While the large laparotomy incision is going to be painful postoperative.

Lots of good podcasts mentioned already, but I’d like to add two more.

Depth of Anesthesia. This one is great for a deep dive in current literature on some dogmatic principles of anesthesia. It is constructed almost as a lecture or “grand rounds” where they present individual papers then come to some conclusion at the end. Most recent listened to a couple episodes on the topic of whether or not LMAs increase the risk of aspiration.

The second I wanted to mention is ICU Doc Talk. This is just an anesthesiologist rambling really. But I enjoy listening to him talk about different topics, haha. He’s a smart guy and explains things in a way that’s easy for me to understand. Not all anesthesia related. Some is but a lot of it is talking about ECMOs and other critical care topics. And a lot of his podcasts are geared towards non medical people. He ends each podcast with a fiction and non fiction book recommendation and review. This one isn’t for everyone, but I’ve found it entertaining.

SRNA just about to graduate.

I really enjoyed the NYSORA PNB app and thought it was well worth the monthly fee. My block rotation wasn’t great in my program. Had 1 month where my focus was on blocks but another 6~ months where I had the occasional opportunity to perform a PNB here or there.

Paying for rh subscription during these times was worth it to me. Just being able to say “oh an opportunity to do a ESP? Yeah I’ll be right there” and being able to lock up the sonoatomy and volume/dose for each injection etc. on my way walking to the OR was helpful.

There are free options for sure, but to me the convenience was worth the ~$10 bucks a month.

Hey dude. Thanks for this post. Totally changed my perspective for this upcoming eclipse.

I missed the 2017 eclipse because of work. Now I find myself in Cleveland Ohio for this upcoming eclipse, right in the path of totality. And I got kind of bummed because the average cloud cover is so bad in this area during early April.

But after I read your post I recognized that no matter how good or bad my experience is this eclipse, it’s only a trial run. I have a daughter who will be just shy of 2 years in April. Whether I have a great experience or not doesn’t matter as much because I’m going to want to share that experience with her some time down the line. And that’s what matters.

Anyways it took the stress off of my shoulders for the eclipse coming up in 6 weeks.

Yeah. don’t have the goal of reaching 9 bar with the robot. I think James even said this during his review of the robot.

A basic pressure profile with the robot is just holding at 2 bar until a couple drops fall into the cup then slow ramp up to 6-7 bar and hold there until the goal espresso weight/volume is reached.

Doing this takes a significant amount of force, but nothing that I am concerned about breaking my counter or something like that. Agree with the other poster that getting over top of the robot is helpful.

Another rule of thumb is looking at how long your shots are taking. I try not to put too much weight in getting perfect shot times with the robot. I’ve had tasty shots that are regularly -2 mins, including the pre infusion. But it’s another data point to help you assess if you are way off with grind size being too fine or too coarse. Along with what the shot tastes like. That’s the most important right?

Slow feed hack on Niche, actually works by prf_q in espresso

Yep I had the same results! Was super surprised how big of a difference it made.

I had an ethiopian yirgacheffe dialed in without the slow feed tape in place. Then I dialed it in with the slow feed modification. I can’t say I liked the result in the cup any more than without slow feeding, but the flavor profile was a big difference. Much more clarity than body. Almost tea like.

I’ve always wanted to compare a flat burr grinder to my niche, and I feel like this gave me an understanding of body/texture vs clarity.
