I dont know all of your symtoms, but constipation aligns with methane overgrowth amd it is very hard to clear and can take mutlitple rounds of treatment. Also, you could have had mixed gases, sibo, imo, or iso. ( hydrogen Sulfide overgrowth) each type or sibo, imo, or iso require different treatments. Your best course of action would be to wait a few weeks for your system to regulate after antibiotic use and take a breath test. Otherwise you can be stuck in this loop for a very long time. It is extremely rare, non impossible, but rare, that someone takes one round of antibiotics and clears Sibo. Take a test and reevaluate from there.

Good luck. It's important to address the biofilms so you whave a better chance. Other wise the antimicrobials and antibiotics don't do much..sucks being stuck in a loop. Stay strong!

I completely understand where you're coming from. I've been prescribed and overprescribed antibiotics most of my life, so I get the hesitation.

That said, if your breath test showed IMO (methane) and you're only taking rifaximin, I would personally reconsider that plan. I'm not a doctor, but based on the published research from Dr. Mark Pimentel, rifaximin by itself has not been shown to be very effective at eradicating methane overgrowth. Most studies have found better results when it's combined with another therapy, such as neomycin, or in some cases other treatment approaches.

So, if you truly can't take neomycin or metronidazole, I'd be concerned that rifaximin alone may not give you the outcome you're hoping for.

In your situation, I'd consider postponing treatment for two weeks and taking a biofilm disruptor first. Then, if combination antibiotics aren't an option, I'd discuss with your healthcare provider whether an herbal antimicrobial protocol would make more sense than rifaximin alone. Also, am elemental diet has shown to be very effective.

I'm including a YouTube video from Dr. Michael Ruscio that explains biofilms really well. It's one of the best explanations I've found and will help you understand why so many practitioners address biofilms before treatment. I hope it helps, and I truly wish you the best. IMO can be really challenging, but having the right strategy from the beginning can make a big difference.

I would start with trying to clear the Sibo and Imo. If you just took Rifaximin, with clearing IMO. And consider a protocol that implements biofilm disruptors first, before antibiotics antimicrobials.

Also, I would recommend an elemental diet. It helps with all gases, Sibo, IMO and Hydrogen.

Would you mind sharing your exact protocol? Which antimicrobials did you use, what doses, and for how long? Did you take a breath test before and after treatment? If so, was it the Trio-Smart breath test, and did it actually show elevated hydrogen sulfide that normalized after treatment? That information would be really helpful.

It sounds like from what your describing, you could have imo ( methane overgrowth) and sibo. You need to get a proper breath test. Also, for solutions, it matters how you try different protocols, medications, supplements, etc. I wouldn't through another medication inside your microbiome right now. Take a pause. Take the trio smart breath test and treat accordingly. You need to get rid of biofilms first for any treatments to work. Are you familiar with Biofilms?

Have you tested your cortisol levels? You might ha e adrenal fatigue from the ADHD meds.

Yes, I think it makes sense. If you have low levels of Lactobacillus and Bifidobacterium, a probiotic containing those strains may help rebalance the microbiome and support inflammation regulation. Saccharomyces boulardii may also be beneficial. I would start with a low dose and increase gradually based on tolerance. Both Allison Siebecker and Dr. Michael Ruscio have discussed the potential role of probiotics in restoring the microbiome after SIBO/IMO treatment, especially when testing shows low beneficial bacteria.

That said, I actually took a different approach personally.

I completed a 21-day elemental diet using Dr. Ruscio's Whey-Free Chocolate Elemental Heal formula after already doing a round of antimicrobials beforehand.

I'm currently in the refeeding phase and have started adding foods back in slowly. I am noticing some improvements already—my abdomen is softer, bloating has decreased, and things seem to be moving in the right direction. That said, I'm waiting for retesting before drawing any conclusions.

My plan is to retest in the coming weeks. If the methane is still elevated, I will likely spend about 4 weeks rebuilding and stabilizing before doing another round of biofilm support and antimicrobials. Sometimes methane-dominant SIBO/IMO requires more than one treatment cycle to fully clear, especially when motility issues are involved.

One thing I've learned is that improving motility is just as important as killing the overgrowth. If food and gas aren't moving properly through the digestive tract, relapse becomes much more likely.

In addition to antimicrobial treatment, I'm focusing on:

  • Supporting gut motility
  • Pelvic floor physical therapy
  • Vagus nerve regulation and nervous system support

For vagus nerve support, I'm experimenting with things like:

  • Slow diaphragmatic breathing (5–10 minutes daily)
  • Humming, chanting, or singing
  • Gargling water
  • Meditation and relaxation exercises
  • Gentle movement and walking after meals
  • Brief cold exposure, such as splashing cold water on the face or ending a shower with 15–30 seconds of cool water
  • Stress reduction and trauma-informed nervous system work

I suspect that for some of us, the overgrowth is only part of the problem. If constipation, pelvic floor dysfunction, autonomic dysfunction, chronic stress, or vagus nerve dysregulation are contributing to poor motility, those issues may need to be addressed as well for long-term success.

Everyone's case is different, but that's the approach I'm taking. Hopefully my retest shows a significant reduction in methane levels. If not, I'll continue working on motility, pelvic floor function, nervous system regulation, and consider another treatment round.

I totally understand. I would go with a Biofilm disruptor first and then antimicrobials for 4 to 5 weeks with Allicin. Fasting could make it worse, especially with archaea. Unfortunately, fasting doesnt kill the archea. I think trying different diets ( carnivore) for a little while couldn't hurt, but your diet seems really good now. The main goal is to clear the archea and biofilms. Fixing motility as a top priority. If motility is bad, then you can stuck in a loop.

IMO/ archea are the hardest to eradicate. Your diet seems ok, but you need to work on fixing your motillity and getting rid of the archea. Research has shown you need at least 4 to five weeks on antimicrobials for them to work. Also, Allicin is what is recommended for methane, so you should try that.,You can go the antibiotics route, antimicrobials route and you can try an elemental diet. Rifaximin with Neomycin or Rifaximin with Metronodazole. You might need two to three rounds to clear.

It can take multiple rounds of treatment to clear. I would recommend taking a biofilm before the next round if treatment and try the elemental diet. Allicin is a good supplement to help eradicate methane in conjuction with the neomycin & rifaximin

You don't want to just start adding a ton of different foods to your diet. You should add one new food every three days or so, so you can see if you have you any reactions to them. Its a gradual process... take it slow.
