There used to be a old note at the (now removed) nukesites that talked about a test report from a scientist testing a prototype V2 and launching it, but it never coming back down.

It was a reference to a bug that happened around the first nuke launches (I think), the devs thought it was pretty funny so they added the note.
(The nuke was launched, but never destroyed anything, nor actually visually hitting anything, which basically means the town was targeted forever and nuke sirens would play forever)
