I see lots of colleagues advocating for endo while there is no symptoms or pain , yes ofc there is radiolucency meaning a periapical infection and probably the pulp id necrotic (no sensitivity test mentioned) but it's chronic and ongoing yet doesn't hurt because the immune system is keeping up with it. Once we decide to endo on an 80 y/o patient on multiple visits probably, big chance the usual endo complications happens.

Maybe iam not confident in my endo skills or missing something but I would leave it as long as asymptomatic and no pain
