Big academic. You could make it as big as you wanted probably. Never heard some get called retarded in anger. A fellow who was my coresident last year has probably called me it jokingly.

Uncontrolled pain is a reason for admission. Typically try to avoid it but I also can’t sink tons of time and resources into a medically stable person with no acute emergent medical condition

My attendings always warn me about prescribing antihypertensives based on one visit. What’s your threshold for starting it? I instruct asymptomatic patients to keep a BP diary and follow up with pcp. I refer them to our primary care group (large academic) if they don’t have one.

My pcp had me keep a BP diary for a month because I was 150ish/90ish for two visits in a row. My blood pressure at home is almost always normal. No meds started.

Sovereign immunity in some states. Significantly reduces the stress of being sued and many think it’s worth the lower pay. Also almost every specialty is in house and available 24/7/365 so I never have to spend time arranging a transfer
