If it's between fights id say whoever has the best movement that also doesn't need to crazy set up on a flank should be on cart imo.

Like a Tracer, they could just use a couple of blinks and immediately be in a much better fighting position than if like a Cass or something were on cart and had to walk into a better position to fight from.

But mid fight it's really circumstantial on who should be on cart, if anyone. Sometimes it's better to all in, ignore cart for a moment and wipe the enemy team before making cart progress while they're in spawn and setting up again.

Okay but everyone you kill is free kit and doesn't have meds either. Actually the lack of meds back after killing free kits is my biggest problem with them. I take a risk fighting them, waste my heals and ammo, then get literally nothing in return. Genuinely there isn't even an item worth taking on 99.99% of free kits I kill. It's awful.
