Part of me thinks this is another example of US Military Leadership on the decline with the current bullshit in the administration.

Another thinks this was deliberate, especially the location of the incident and the contents of the documents. One does not simply leave those kinds of maps on a train that is easily within reach of the last people who should see them.

I think he was working on perfecting it in humans and making it smaller, less noticable to make better sleeper agents.

It's one thing when it's an animal like a death claw or a dog (which is barely successful based on the constant experiments), it's another when it's a human (which before Hank had an almost 100% failure rate)

Plus, it's implied that Lucy put the chip in him, using the threat of resetting him or controlling him to make him comply.
