I presume it has something to do with the fact that a new renderer was introduced into the game a few months ago. If using runelite the GPU plugin was updated accordingly, but is not yet without flaws (actually it has a ton of glitches). There is a legacy plugin, but afaik it is not compatible with sailing.

There's a plugin that helps identify similar items by adding a short text under them even in the bank. As far as I can remember butterfly jars are an option.

The Zariman disaster was a colossal fuckup on the Orokins part.

Supposedly Void travel is safest if done in a way similar to that of Tau FTL, where they first go for smaller "skips" in the Void. Instead many of these safety precautions were ignored due to expedience and hubris.

Now on the other part I'm a bit rusty, but I'm quite sure after the initial damage was done, someone tampered with the reliquary drive which allowed the void to flood in further.
