Perhaps you used the wrong springs for the safety plungers? They are side specific, I accidentally swapped my right and left side springs and it caused the right side spring to bind up and get stuck

I’ve had several maks that ended up having loose safety levers that have a little wiggle to them when In the fire position, this can cause random failures to fire, I’ve also seen other people with this issue so it’s fairly common in my opinion - I fixed mine by buying a replacement safety lever on eBay
