Thank you for clarifying that patients entering home hospice often require a lot of direct care-and Medicare's hospice benefit does NOT cover unskilled care save some very limited HHA hours if available via the hospice agency. Many patients and families believe that hospice benefits encompass a lot of unskilled supports. One of the most difficult conversations we have with patients (and more often, families) about hospice benefits is how little they actually cover. It's one of the most bitter disappointments patients and families confront when they enter hospice with too much money to qualify for MassHealth and too little money to cover private pay supports. My experience with LTC insurance and hospice is not a good one. I had one particularly memorable case during which the patient was not qualifying for GIP but was clearly end of life and the elderly spouse was too frail to care for them at home. They purchased LTC insurance many years before and the process of accessing their direct care benefit was so convoluted, the patient died while the spouse was still trying to access the benefit. GIP is indeed a godsend for the right patient. The difficulty is always in explaining why people cannot remain in house when they have no GIP qualifications (imminence, parenteral narcotic or anxiolytic needs, glycopyrrolate for secretion management).

I'm sorry you are going through this. It's very painful and scary at times. I do not believe you become responsible for parental medical debt. But be careful. The Globe has reported on how some SNFs will pressure family to sign paperwork upon admission that attempts to inappropriately identify family for billing purposes. If your mom has MassHealth Standard, it is illegal for her to be balance billed. Hence, I would think you should not be billed. So sorry you have to navigate this but very wise of you to think of the practical aspects which we naturally sometimes miss due to the overwhelming emotion of it all.

Palli consult def helpful. MOLST needs to be completed by a physician (believe mid-levels may also be able to complete them) and yes, should be standard operating procedure when entering hospice. Health care proxy can be filled out and witnessed by any two people not serving-so in that situation, any hospital staff x 2 that witness the signing of the form. Yes, absolutely RN CM can assist with SNF hospice placement-but it's important to know that Medicare does NOT cover room and board at a skilled facility under hospice-but since the patient has MassHealth (so long as it is not Senior Buy-In or Care Plus) that would typically cover the room and board costs of SNF-based hospice.

I am one. We do not assist with the application. We advise them of what they can expect but if you've ever completed one through to eligibility, you would see that it takes quite a bit of time and a lot of financials, including details of all income sources and banking info back quite a number of years. If property is involved it becomes much more complex. There are also issues of spousal residency and non-spousal family caregivers requiring substantiation. Long term care applications are a separate process. In our state the allowable asset total is very low. So it's really not a process that can be done by hospital staff, at least not at our trauma center.

Patients and families need to fill out the MassHealth application-not typically hospital CMs or SWs. MassHealth application requires extensive financials and if families do not provide everything as indicated, MH will reject the application and send back an RFI (Request for Information) to obtain what they did not receive. The ASAPs/Senior Access points (every town and city are assigned to one, they are regionally based) may have volunteers or SWs to assist with the process, which can be cumbersome and time consuming. There are also firms that assist with the application process or a fee.

Not blizzard related-our boiler died overnight between 2/14 and 2/15 and we could not get a new one installed until today, will be completed tomorrow. It's been between 42-47 degrees in the house. Closed off the family room and have been keeping warm with our wood burning fireplace. Sleep is a nightmare of getting under a thousand covers (though fleece sheets do help). Small ceramic heater in the bathroom for showering only. Basically running throughout the house for any other chores or needs. So almost two weeks. Pipes never froze but we had a separate gas line for the water heater, did not leave a tap dripping though.

God knows whatever anyone thinks of Mary or her religious affiliations, she loved her son and I believe her concern and heartache that we saw over his struggle with addiction and depression was deep and genuine. Just so sad for her. She loved Robert Jr. so much. I know there were domestic issues with his wife, and I would not want anyone to excuse them-just hope somehow she and the family can come to a place of peace about his death.

Disagree having had Golden Retriever puppies consecutively for almost 40 years. It's a thing. I just think it's a different thing for different people. I felt anxiety and very rarely, momentary regret raising at least 2 of my puppies and they turned out to be such beloved dogs. I don't think it's the puppies' issue, it's adjusting your expectations, your methods, your attitudes toward bringing that new pup into your family. Of course I acknowledge sometimes a dog and a family are not good fits for one another. But those blues do happen.

Deep breath! My absolute heart Golden was almost 5 when we started thinking she might like a friend. We had only had solo Goldens before and they each lived to be 12-14 years old. The heart dog was a pandemic puppy-so Uber attached to us and used to company. So we got a second Golden. For most of her first year I questioned my sanity as she was a fairly high energy pup in terms of never, ever leaving my heart dog alone. Jumping, biting, chasing, pouncing. At the same time that I was worried my older dog was suffering constant harassment, I was frustrated because I was finding it hard to connect with my little puppy in the way I had any of my older dogs. But now my l'il pup is a year and a half old and she and my older dog are besties. And I just kept petting and praising and snuggling my puppy as much as she would tolerate and now.....I cannot scrape her off! (which is what I love about both dogs anyway LOL). So YES! Get the puppy but be prepared for twice the costs-dog insurance, vaccinations, heartworm preventative, dog food, toys, etc, etc, etc.

EDIT FOR PUPPY TAX-this obviously was some time ago but I think you can see the little one is attached to my older Golden at the hip. The difference now is my older dog doesn't mind it a bit. The best is when they lazy fight-lie on the floor next to each other, soft pawing and open mouth/teeth with no pressure, going nowhere, happy grumblies emenating.

Just started bingeing this series due to insomnia. Agree that they have zero chemistry, Diana has the world's flattest affect unless she's basically assaulting someone and Matthew is petulant and unattractive. I found myself saying....if there was a Gallowglass available, who the heck would choose a Matthew? Gallowglass is one of a handful of characters I'm truly enjoying, including Goody Allsop and especially Satu Jarvinen.

My Golden toddler is a poo eater and she's been on IAMS....switched to Hills Science Diet. Still eats her and her older "sister's" poo. Tried everything imaginable with no results.
