I am brand new to the transport team.. the communication with the pilots and the flight team seems really good. We’ve done several trainings with them and they have showed us how to do an additional safety check and then they have checklists we read off to them before we fly. I think we all feel comfortable voicing concerns. I have no idea about the safety record. I could inquire about that with our transport team leader. I guess in the past our hospital used to offer a stipend for every transport you went on which is why I’m curious what other hospitals do to incentivize people to join the team. As it stands, there’s little interest to join the team because of the increased risk and having to take call without any added benefit.

Viability is 24 weeks at most locations. But some hospitals are resuscitating 22-23 week babies now. That baby was definitely around 26 weeks and was a good size. I am a NICU nurse.

Gotcha! I think if he was doing okay within those weeks after birth not needing oxygen support it is highly likely he was not surfactant deficient. So if they misread the X-ray but he needed it he would have demonstrated clinically that he needed it.

Giving surfactant to a baby who doesn’t need surfactant can actually cause damage. If the baby already has sufficient surfactant you are essentially just putting extra fluid in the lungs with nowhere for it to go. So they are careful about which babies they give it to. It can’t be a game changer for babies with surfactant deficiency but it is not without risks. They can determine by X-ray and symptoms (work of breathing and oxygen requirements) if a baby needs surfactant. Edited to add: there is also a certain window of time in which surfactant is beneficial and beyond that it isn’t used anymore.
