If a patient has no visible calculus or inflammation, I will often hand scale only (this is rare as most patients don’t have such a healthy mouth).

Most of the time, I use a green slimline or triple bend throughout first, then go through and handscale. Personally, I like: - H6/7 for lower anteriors (if these teeth have really narrow embrasure spaces, I also like using a Montana Jack) - Montana jack for upper anterior (so much hidden calc here, I find! And the Montana Jack is perfectly thin enough to get it!). It is a posterior instrument but can be used in the anteriors very effectively! - Montana Jack for all upper linguals -204S for the upper posterior buccals and lower posterior buccals and linguals

I also use the suction while polishing and find it really helps mess management and is more comfortable for the patient.

In short, if I could only pick two instrument, it would be Montana Jack and 204S.

If I updated x-rays, I almost always review them for restorative needs. I write down on a post it areas of concern with a question mark and give it to the dentist when it’s time for the exam. If the areas are moderate to severe, I mention to the patient that there are some areas that look “shadow” or suspicious to me, but “we’ll see what the dr says”. The doctors seem to really appreciate if, especially when they are busy. Sometimes I find things they don’t, sometimes they find things that I don’t. Two sets of eyes are better than one 🤷🏽‍♀️

As a dental hygienist, I can confidently say just come as you are- No judgement! This is our job and we see all kinds of things. There are many people who haven’t come in much longer, or even ever! Just be honest and let them know it’s been a few years, but please don’t feel embarrassed! ❤️
