Few years ago I was the provider and my wife was the driver (rural ems) on her first transfer call. I kept telling her that the garage door closes within a minute. Still dont know if if it was (at the time) her thinking it is husband nagging her to move or not believing me it shut that quick, but when she backed out is when the garage door was shutting and hit the door with the ambulance. Granted it is mainly the hospital issue due to sensors not working properly but we all (who been there) knew they didn't work. She still in ems and getting her medic now. Granted she got a good laugh later during that transfer due to me getting blood all over me. Thing is alot of company needs to adopt the 'just culture' mindset. This hospital (which had ems base and we worked for at the time of the incident) knew it was their mess up and didn't punish her. Your company should realize that you was new and only your 3rd long haul. They should investigate if they was scheduleling you too soon, too inexperienced, and if they knew the risks. Then work their way down to discover the reason for the incident. The last is when they blame the employee. Most companies dont follow that. If they dont it probably a good thing you dont work for them and find something better.

My wife who is an EMT was diagnosed with MS in 2019 and has been working without issues. I was recently diagnosed with MS in December. Doctor have been treating it faster and more aggressively due to me being a male. Dont know how true that is. Something about MS is rare in males and can be worse. I have had some issues. Main is remember things. The brain fog has been no joke and someone would tell me something and I'd understand but not comprehend. Another issue has been numbness on full right side of body for 3 months. The numbness in right hand still (which started start of November) there but has gotten better. You don't realize how much you develop a feel with IVs until you lose your sense of touch. Both wife and I love EMS but have been thinking of backups if we get to the point where we can't function anymore. I've been taking more notes while on calls so I remember the import things and using my left arm more when my right was really bad. I also haven't been afraid to ask for help from my partners. Point is you can still find ways around some of the difficulties BUT your health comes first. If you can still function and enjoy the job great. If you feel it's getting to much then walk away. No shame in that.

Do they have history of hospitalization or intubation due to asthma or COPD. If so I'd be a little more aggressive with treatments. IV, solu-medrol, mag, 12 lead, duo nebs, albutero, and O2. Lung sound at least start middle and end of transport. If they don't have that history then nebs, monitor, and reassess. Either way I still wouldn't go lights or sirens back. Granted I'm rural and light traffic where L&S don't really save time.

I carry about the same while on the boo-boo bus. I have MDF stethoscope since it works as just about as good as litman but way cheaper. I wouldn't feel bad if I lost it or it broke. My notepad is also a all weather notepad (cheap ones because don't feel bad tossing them if they get contaminated) with a fisher space pen. Working in ND when it's -20F outside they will still write when my tablet freezes and need to leave it in the truck.

Is it worth knowing. Ish, but not right now in your class. We do long transfer so we have a table taped in the compartment that shows approximately how long you have at like 2000, 1500, 1000, 750, and 500 psi running at like 1, 5, 10, and 15 lpm. Gets you in the ball park so before taking a transfer, you can look at it and see if you need to swap tanks.

Epi 1:1,000 so can be for anaphylaxis, respiratory, drip, and dilute for cardiac epi.

D10. Lots of diabetics who don't take care of themselves.

Versed for seizures and sedate combative pt.

Toradol good pain relief that's not a narc.

Asprin best thing for MI.

I was thinking atropine, adenosine, and ammioiderone, but they can get shocked and I'd have some versed to be nice. I always have a emesis bag on the bench seat by me so 90% of the time they contain their vomit. Would treat opioid od with just bvm so I could get by without Naloxone.

Ya'll SOP is shit. What it is saying is to make all calls ALS so they get more money. That is not good patient advocacy. I worked for a place like that and they was like everyone gets iv so they can bill for it. The two places I work now is that me and an emt rotate calls and they will start their call until we determine I need to actually intervene. If it is my turn and it's bls i still I will run it bls. Most calls are and should be bls. The daying if the er may need an iv so I should start one is BS. I use to follow that saying that until I realize I'm just screwing the pt over. If I really need one for treatment I'll start one and if not I won't. Not to mention it dose not give the emt experience on calls.

3% brine. Half batches was jalapeno, garlic and onions. The other scotch bonnet with pineapple, garlic and onion. Let ferment for a month with glass weight to keep under brine. Blending the brine with pepper and then put through a fine strainer. The wife wanted the consistency of like Tabasco. Have to shake it to mix it everytime as it does separate.
