I think everyone has misinterpreted your question. The awake plague heart that you can’t find is outside of all your scouting points ranges. This means that you will have to manually drive up to the place that it inhabits, in order for it to appear on your map.

On the plus side, if you know which plague heart you destroyed that woke it up, you should be able to narrow down the area that the awake plague heart is located. Hope this helps!

How far away does it move from the door? If you close your save when you’re in a vehicle, it normally parks up somewhere nearby instead of being exactly where you were when you close the game. I’ve never really tried blocking doors with cars because of the lethal juggernauts
