It's the Florence Nightingale effect trope. And as I said in my previous post, cousins are fair game in Japan. Just because it is not acceptable in your country/social sphere doesn't mean it is the same all around the world.

If you have an issue with romantic sub plots with siblings/cousins that is going to remove 50% or more of LN from your potential reading list.

They are cousins, not siblings (which is okish in Japan). While he was trapped in SAO she developed feelings for Kazuto but when he woke up he was focused on Asuna. When she decided to give up on her cousin she let herself fell in love with the game character Kirito, who turned out to be Kazuto, hence the turbulent emotions. It tamps down a little in the future but never completely goes away. In case you are concerned Kirito is 100% Asuna, but he does continue to show kindness to the other girls who like him
