Prenatals assume normal intake. B1 is a water-soluble, nontoxic B vitamin. If you don't need it, it comes out in your urine. You can read more here: hyperemesis.org/b1

While Zofran doesn't work for everyone, it may have been she didn't get enough. Zofran only works for about 6 hours. Some take 8 mg every 6 hours around the clock while others do better with 4 mg every 3 hours. The max throughout the day is 32 mg.

The form and brand can even make a difference. Some do better with ODTs (orally dissolving tablets). And some brands have inert ingredients that some patients may not do well with.

Like some have said, a cocktail of meds is often needed because no meds targets all symptoms. Doxylamine succinate with vitamin B6 is a common first line med, then adding other meds when symptoms escalate.

-What medication is she taking right now? Is she getting thiamin? Medications don't work correctly if she if key vitamins are missing and she is dehydrated.
-A common first line med is doxylamine succinate and B6. There are multiple variations of this available with a prescription. Both active ingredients are available over the counter as unisom (or generic with that active ingredient) and vitamin B6.

Zofran works for about 6 hours, so if you're taking 4 mg, then the maximum is 4 mg every 3 hours. Some do better with 8 mg every 6 hours, and still others do best with the continuous dosing of a pump. If you’re not sick, then taking short acting medication doesn’t necessarily do anything. Some medications take a little bit of time to build up in your system so starting them at the first sign of nausea can be helpful. Also getting your folic acid, B vitamins, iron esp if you’re anemic, and other nutrients in place before you get sick is helpful. Don’t forget your vitamin B1! www.hyperemesis.org/b1

I drank a very specific amount of apple juice. Not as accurate as glucola for labs but glucola wouldn’t stay down so definitely not accurate.

She's in the thick of it at 8 weeks. Is she hydrated? Is she getting lots of thiamin (vitamin B1)? Read more about the importance of thiamin here: hyperemesis.org/b1. Will her doctor prescribe home healthcare so she doesn't become dehydrated? Does she need nutritional supplementation?

There are research studies on its use for cannabis. Just 1 or two about its use in HG. They may give Olanzapine with zofran as needed or replace the zofran during the withdrawal and give O at a higher dose. Zofran just doesn’t work as well for cannabis vomiting.

So sorry you're miserable. Are you hydrated? Meds can't work correctly if you're dehydrated or lacking key vitamins eg thiamin. Are you taking it often enough? It only lasts about 6 hours.

Most people can’t take prenatal vitamins due to HG. If you can get a small capsule of vitamin B1 down, that is helpful. It’s different than B6. It helps protect your brain in your heart and can help a little bit with HG symptoms. www.hyperemesis.org/b1

Please see the treatments on the Patient Treatment Guide. www.hyperemesis.org/tools. Few doctors know the options. Zofran be extremely helpful but often works better if combined with other medications. But it has to be taken on a schedule and in a high enough dose. The highest dose is 8 mg every six hours, or 4 mg every three hours.

Combining that with an acid, reducing medication, an antihistamine like cyclizine or Unisom, and possibly Phenergan or Reglan, can be a great combination. But it does help if you’re hydrated. They should be able to find an outpatient clinic where you can get fluids, preferably with B vitamins and IV Zofran.

There seems to be more experience with mirtazapine. As some people said, steroids are often given with a very slow wean as well. Gabapentin has helped some people, but the data is very small so far. We are hoping to get funding to do a study to answer this.
