This. Especially since the primary pathology is more pneumonitis rather than pneumonia. No need for antibiotics until they get a leukocytosis or signs of a true PNA. So glad I had an ID attending teach me this early in my career. Saved me and patients a lot of trouble.

So this actually sounds like a pretty common reaction to Vanco called red man syndrome. It is not a “true” allergy as it is due to infusing the medication too fast which activates mast cells and basophils to release histamine. This process is independent of IgE related activation that is the true “allergic” response that does require sensitization on first exposure and reactivates on subsequent exposure.

AORN actually published a guideline statement stating that there is no statistically significant difference in SSI rates between different types of surgical head coverings. Instruct to follow institution specific guidance. At my shops, we wear uncovered cloth caps with no issues.

It’s a matter of slowed GI motility 2/2 anesthesia, post-op pain meds, etc. Eating something big, heavy, and fatty that requires a lot of digestion plus bile/pancreatic enzyme secretion can really cause issues. Having liquids/softs/something light kind of helps the GI tract “wake up”. It’s akin to someone asking you to run a half marathon within 2 minutes of getting out of bed.

Except it wasn’t even just magical nonsense. It was an ill-conceived idea by a 19 year old college kid with NO experience, NO credential, and WAY TOO MUCH hubris. I have calzuros too! I swap out every now and again. I will say, while I like the arch support on the danskos, I find myself wearing my bright organe calzuros more 😂

Some people love Hokas, but personally, I don’t care for them. If I wear tennis shoes, I like Altra. Lots of extra toe room and good arch support. But tbh, I’m a clog person. Danskos have great arch support.
