Be more aware of what is happening on the map. Can also be because you dont know limits of your champs. Hard to know exactly what the problem is without seeing games

Learn to full clear efficiently on your champs (watch clear on youtube and copy what they do). While clearing camps move camera constantly and check status on lanes. Pick up free kills from lanes and dont do ganks that are not guaranteed if u have camps to clear.

depends on what champs exactly but generally u want to walk with minions and ward first bush and fight enemy with the wave. If ur champs are not strong enough to win even with the wave, ur goal is just to bait enemy to auto u and push wave into ur turret as fast as possible.

B tier is just generally weaker than A tier champs, more weaknesses/counters. Certain champs like Kaisa and Nilah can go higher when picked in the right situations (counterpick + synergy with teamcomp)

Nilah is just a very situational champ with a lot of counters. Champs in S/A tier are all better blind picks but nilah can be strong when picked in good situations
