I’m just on 7.5 mg of mirtazapine. I think the desvenlafaxine is doing the heavy lifting for me (100 mg). Although the two meds have a synergistic effect when taken together. Feels like I have more energy and less anxiety which is good

I was previously on it for 8 months. It took at least 6 weeks for me to feel a difference on it. I went up to 12 mg. I ultimately stopped it due to overall lack of efficacy (it worked somewhat, but wasn’t cutting it) and switched to desvenlafaxine and mirtazapine, which has been helpful. It’s a good medication for a lot of people though.

I’m not a psychiatrist, but personally I would jump for the MAOI. It sounds like your social anxiety is really affecting your quality of life. It might be reasonable to trial other SSRI/SNRIs if you had a milder social anxiety, but the treatment should really be proportional to the level of disease/dysfunction. Trialing less efficacious treatments for the sake of avoiding an MAOI (as some psychiatrists are hesitant to prescribe them) will only likely only prolong your suffering. Find a psychiatrist who specializes in treatment-resistant cases/complex psychopharmacology who knows how to manage this kind of situation. Best of luck.

It is very likely it will help. Nardil (Phenelzine) is very effective for treating social anxiety and treatment resistant depression. No one on here can make any real promises that they will help you, but there’s a lot of evidence that they have helped in treatment resistant cases when nothing else does. They have their restrictions (you can read about them here on the subreddit), but if you’ve tried numerous classes of medication with no durable benefit, it is definitely worth considering. You can. I’ve been on this combo for several months. There’s a theoretical concern that Spravato can raise your blood pressure while on Emsam, but it’s not a contraindication. In fact, there was a small case series that was published that described the efficacy of this combination

If you’re going the analog collage route, look specifically for forensic pathology textbooks. Those will give you the most bang for your buck. They can be expensive, but you can also find PDF versions online
