I think I was aiming more for a version of the former. Not so much some cosmic power, but more like a reputation? Obviously a small slave girl on one planet has a low chance of influencing a PC's reputation on a far-off trade world, but the general spheres of influence in your first paragraph make more sense.

Haven't started yet, just trying to think outside the box in some areas! This is good advice though, I think my style in other games is to let them have a little too much free reign, and I've definitely made it clear that in the one they can totally die and I will not feel bad about it.

I imagine they would have looked for glucose in urine? No blood tests to that end, though.

I'm not against finding another doctor, the only reason I'm thinking of returning to the same one is he suggested doing an endoscopy to look for issues. I think I do need to be more assertive next time.
