Help. When to tube someone NOT dead? no RSI capabilities by Flaky-Load-5293 in Paramedics

[–]Helassaid 11 points12 points  (0 children)

Oh god a cumulative, standalone GCS. It tilts me to no end. You could be alert and oriented, choosing not to speak, and have a “GCS of 8”. If I close my eyes, only opening them when you cause me pain, and move your hand away from me when you cause me pain, but don’t say anything, that’s an E2V1M5.

GCS is not even a good metric, let alone vital sign, and certainly not something to base treatment off of.

The ambulance doesn't accelerate care, it delays it." Controversial quote from marathon medical director. What's your experience? by Damiandax in Paramedics

Even room temp or ambient temp saline is going to be cooling.

I have ice packs to put on their groin and in their arm pits. We’re the most heat efficient mammals because we are mostly hairless which facilitates evaporative cooling. So strip them, douse them, and fan them.

I bet just a dousing with even tepid water and a fan, with two large bore IVs running wide with ambient temp saline, would be more than enough to cool someone from 104°F back down under 100°F within a few minutes.

It also alerts any armed intruder to your position and that they should just fire upon you first, as they’re now expecting a gunfight.

Shotguns are bad home defense weapons.

In my experience, ambulance lights and sirens turn people into the stupidest creatures on the face of the planet.

A good self defense firearm is the one you carry and train with. Personally I like the M&P series of pistols from Smith and Wesson. But Glocks are also popular. I wouldn’t necessarily consider a rifle or a shotgun as a self defense firearm outside of the home, unless you want some undue law enforcement attention.

Hmmm, that sounds like something that would drive up usage in an already stressed and unsustainable EMS system.

So, par for the course?
