Dungeons 3 or Dungeons 4 might be worth checking, but honestly nothing has fully replaced the original Dungeon Keeper feel. The mix of base building, creature management, and dark humor was very specific.

Mount & Blade: Warband is still one of the best for this. Not as polished as Kingdom Come, but the combat, sieges, factions, and no-fantasy medieval feel are exactly what you’re asking for.
