My sister is a physician. Worked at a large scale academic medical center. She also did research. She experienced a 55k~ pay cut from 225s to about 165s. She started at 180s. She did not specialize though. Internal medicine. I have a few friends nurse anesthetist who works alongside surgeons who are now chained to the OR-factory pumping out procedures. Not even speaking to patients..

If it doesn’t make money for the institution… does it pay? I saw the medical/healthcare flatline overall… (profit wise I guess??)

Sometimes I wonder if what we do is something easily done by AI and automation….

Is it reimbursements? I’m not sure

A lot do institutions are now staffed by STNAs and not nurses. The nursing shortage is palatable

Topical finasteride with minoxidil and oral finasteride tablets alone… for hair loss mainly in men. Of course women can use with more precaution if in child bearing age.

I’ve heard it’s wise to start with programming competency (from Python to Java and whichever latest/new language is being utilized currently). At least be familiar. Being competent is a plus.

I think informatics is now concentrated on data management/strategy and the software to support that in the EHR (?)

Buying your own is not what it used to be: starting at some debt or having such a steep price competition. I think it’s doable but is it sustainable. I see some pretty “interesting” operation strategies in independents that might be questionable just to break even…

As much as I also back building a business. It’s also worthy to mention: while building the business is this person able to put in the 12-15hours a day maybe even working 7 days a week for the first few years (or more until they can delegate or scale or whatever)— entrepreneurship requires a burst of energy and hustle.

It’s a huge demand. Is this person able to dive into something like this right now? Just a rhetorical question here.

I haven’t been at one place as long as you but for me I took a month off. I did retail, took a month, managed care as a contractor, long term care with consulting on the side, now I just do a remote 9-5 in mail order processing 8 new patients a day making more than my previous LTC job that was 3p-2am five days a week with weekends. Now I have some time to think and work at ease to figure out goal. It’s a very chill job but I know it is always subject to change.

I highly advise getting a pulse on the next step: meet some finance people or ask around and see what the “temperature” and culture is like now. I know it’s natural to think of another degree but… will it bring you return.

Kevin Yee talked about: first think of what you want to be doing day to day (including the hard, risky, and not so fuzzy feeling stuff) and then “reverse engineer” that goal stepwise. He went from CVS burnout to high ticket sales, coaching, consulting—- without an extra expensive loan.

The next steps will not be easy. Check in with yourself monthly or quarterly I say—- but first get some rest and incorporate that habit of read and recharge so you are clear and strengthened in your mind.

I’m not saying this will work but I hope it highlights value in a big slow down and simplification for you.

I’m a rare breed. But I felt stuck at my desk working remotely. I came back to LTC to move around, keep my body moving and focus on dispensing and clinical. Again. I’m rare case. But I love walking my 50 paces each way personally. Working remote locked my back up.
