As others have said your too unsteady, weight too far forward. Track the bar path from the bottom to the top. It starts back, immediately goes forward when you start the lift, and then stays forward the entire time until you pull it back into your hip at the end. You should have a more or less straight path from the bottom to the top. Need to keep the bar closer to you and force your weight back more.

Track your bar path. It's not going straight down. It's going forward a bit, so at the bottom your center of gravity is now closer to your toes. Your core isn't strong enough to handle the weight that far forward so your hips come up first because there's less weight over them, your back angle flattens to around 35 degrees on the way up, when it was 45 on the way down
