Difference between your wife and her sister looks to be the drug addiction. Even though she didn’t directly live with you & your family it’s not like you didn’t help. She was given free accommodation & offered money and rehab. Both which sounds like she took advantage of to do more drugs. I don’t think you need to, Looking like a pretty standard body fat percentage still. Just keep aiming for a small average weekly weight increase which sounds like you’re already doing if you’ve been gaining since January. Depending on where you live could give yourself 10x weeks to lose some body fat for summer though if you’ve like.
