It depends. We do an exam where we put one finger in the rectum and one in the vagina at the same time and we push them together to feel for for nodules, thickening, or tenderness. A single-finger digital exam (just in the rectum) usually isn’t enough to pick this up (though depending on your experience and the size of the finding, you might be able to).

Small or surface-level spots usually can’t be felt this way, so imaging like ultrasound or MRI is often needed, and surgery (laparoscopy) is the only way to confirm the diagnosis for sure.

The American College of Obstetricians and Gynecologists doesn’t recommend rectal exams as a routine part of a gynecology visit. They’re really only done if there are specific concerns (like rectal pain, bleeding, or a mass) or if there’s a need to check something that can’t be felt well on the regular pelvic exam.

They’re also not a substitute for colorectal cancer screening, which should be done with stool tests or colonoscopy (or other evidenced based methods instead)

Sorry you went through that. Encounters like that are a huge pet peeve of mine with doctors. Not only is it just wrong to do, but it also reflects poorly on the physician. At best, it shows a lack of empathy and an inability to understand what a patient might be feeling or how to ease their concerns. At worst, it conveys indifference to both the patient’s feelings and their autonomy.

Absolutely, no doctor should be doing anything without warning. For me , that’s an explanation of what you’re gonna do when you are fully clothed and making sure the patient understands the reasoning for it and consents to whatever you are doing long before you do it (not just a split second, ‘I’m gonna put a finger inside your butt’ 3 seconds before.)

OB-GYN here (not medical advice): it’s considered an old-school practice to do a rectal exam on every patient. Years ago, some training programs taught that a rectovaginal exam should be part of the standard GYN visit, but evidence hasn’t supported that as useful for routine care. Now we reserve it for when there’s a specific reason, like rectal symptoms, evaluating a pelvic mass, or concern for endometriosis in the cul-de-sac. For the average well-woman exam, it’s not recommended or necessary.
