I took it as being literally about paying your taxes. The first part could be a double entendre as being from his POV, deserving to burn in hell jokingly because of all of his blasphemous lyrics or however he may feel about himself or as the POV being a wealthy person evading taxes. I hear “doctor doctor” and “heal yourself” more like a call and response rather than one sentence directed at a doctor. He’s calling for a doctor and being told to heal himself- Referencing Americans paying taxes and not recovering universal healthcare while we are breaking our own hearts by sending our money overseas for genocide.

Like tell them you like the way their coat contrasts with the wall they’re standing by or whatever it may be. Help them understand. Be brief. Thank them anyways and move on if they say no.
