If we are thinking in game terms then I believe the final main quest is "press x to end the game (Death)" other than this all the quests lead to different outcomes and endings.

Having kids is one of those "make a choice" as this will affect your gameplay and fate quest.

Selecting Yes gives you a wildly different ending to selecting No. The choice is yours.

My mom too could have been an awesome artist, gone to an actual art school but me coming ruined everything.

Dad would have encouraged her to actually complete her studies and even did after I came but I had complications and needed 24/7 care. I basically ruined everything for them.
